频域OCT量化参数分析特发性黄斑裂孔患者术后视力预后

摘　　要：目的 探讨特发性黄斑裂孔（IMH）形态应用频域OCT量化,评估患者术后视力预后与量化参数的相关性.方法 回顾性研究,收集2012年3月至2014年6月在潍坊医学院附属医院眼科诊断为特发性黄斑裂孔行玻璃体切除联合视网膜内界膜剥除术,术后裂孔闭合的患者24例（24只眼）,其中男性6例,女性18例.根据Gass分期：Ⅱ期8只眼,Ⅲ期12只眼,Ⅳ4只眼.手术前后均进行最佳矫正视力（BCVA）检查,利用SD-OCT测量黄斑裂孔的底径及最小径,裂孔孔径两端与底径两端之间的距离,通过公式计算求得黄斑裂孔形成因子（HFF）;并对患者术前与术后最佳矫正视力进行非参数检验,HFF值、黄斑裂孔底径分别与术后最佳矫正视力进行相关性分析.结果 术前患者平均BCVA（1.24±0.43） logMAR,术后平均（0.74±0.36） logMAR,较术前有明显提高,差异有统计学意义（Mann-Whitney U检验,P=0.002）;HFF数值与术后BCVA呈负相关（r=-0.633,P=0.024）;裂孔底径与术后BCVA两者无相关性,差异无统计学意义（r=0.370,P=0.090）.结论 在术前对患眼进行SD-OCT扫描结果量化参数,是评估IMH患者术后视功能的一个较好的参数指标。Objective To find the correlation between the quantitative parameters of SD-OCT and postoperative visual acuity of Idiopathic Macular Holes.Methods Collected from March 2012 to July 2014 in Weifang Medical College affiliated hospital of ophthalmology diagnosed of idiopathic macular hole, in patients with closed 22 cases （22 eyes）, 6 eyes of men, women 16 eyes;According to Gass stages： 8 eyes at stage Ⅱ, 12 eyes at stage Ⅲ, 4 eyes at stage Ⅳ.Best corrected visual acuity （BCVA） were performed before and after surgery, macular holes were measured by SD-OCT, and calculated the hole form factor （HFF）, nonparametric test was performed to understand the BCVA with preoperative and postoperative, correlation analysis was performed between HFF value, macular hole bottom diameter, and the postoperative best corrected visual acuity.Results The patients＆#39; average preoperative logMAR BCVA was （1.24±0.43）, averaged postoperative logMAR BCVA was （0.74±0.36）, obviously improved than preoperative one （the Mann-Whitney U test, P =0.002）.HFF numerical and postoperative logMAR BCVA showed a negative correlation （r =-0.633, P =0.024）.There was no correlation between bottom diameter and postoperative logMAR BCVA （r =0.370, P =0.090）.Conclusions With SD-OCT scanning, HFF may be the predictive factor for a good visual prognosis after IMH surgery.
