喉鳞状细胞癌组织Twist表达与上皮-间质转化相关性分析

摘　　要：目的研究喉鳞状细胞癌及癌旁黏膜上皮组织中Twist蛋白的表达,分析其与喉鳞癌上皮-间质转化及侵袭转移之间的相关性。方法采用免疫组化EnVision法检测湖州市中心医院2005-01-01-2010-12-30资料完整的74例喉鳞癌组织和30例癌旁黏膜组织中Twist、E-cadherin及N-cadherin蛋白的表达情况,统计分析三者之间的相关性以及与各临床病理指标之间的关系。结果喉鳞癌组织Twist的阳性表达率为62.2%(46/74),明显高于癌旁黏膜组织的13.3%(4/30),χ2=20.388,P<0.01;喉鳞癌组织N-cadherin的阳性表达率为58.1%(43/74),明显高于癌旁黏膜组织的20.0%(6/30),χ2=12.441,P<0.01。39例喉鳞癌组织E-cadherin蛋白的表达明显减弱,而癌旁黏膜上皮均正常表达,两组间差异有统计学意义,χ2=21.802,P<0.01。Twist与N-cadherint蛋白表达正相关,r=0.527,P<0.01;与E-cadherin蛋白呈负相关,r=-0.433,P<0.01。Twist、N-cadherin蛋白过表达和E-cadherin蛋白低表达与喉鳞癌组织分级、T分期、TNM分期、甲状软骨侵犯及淋巴结转移具有相关性。Log-rank分析显示,Twist蛋白表达阳性喉鳞癌患者的5年生存率为21.7%,明显低于Twist阴性组的39.3%,P<0.01。结论 Twist在部分喉鳞癌中表达上调,并且与肿瘤的侵袭、转移以及不良的预后相关,其促上皮-间质转化作用可能是重要机制。Twist蛋白检测对喉鳞癌患者预后的评估具有重要的参考价值,其基因也有望成为喉鳞癌的潜在治疗靶点。OBJECTIVE To investigate the expression of Twist in laryngeal squamous carcinoma and its relationship with epithelia-messenchymal transition（EMT）,invasion and metastasis.METHODS Seventy-four cases of laryngeal squamous carcinoma specimens and 30 cases of tumor-adjacent mucosal tissues were collected,and the expressions of Twist,E-cadherin and N-cadherin protein were detected by immunohistochemistry Envision methods.The results were statistically analyzed with the clinical pathologic data.RESULTS The positive rates of Twist and N-cadherin protein were 62.2%（46/74）and 58.1%（43/74）in laryngeal squamous carcinoma,which were significantly higher than those in tumor-adjacent mucosal tissues（χ2=20.388,P〈0.01;χ2=12.441,P〈0.01）,13.3%（4/30）and 20.0%（6/30）.Down-regulation of E-cadherin protein expression was found in 39 cases of laryngeal squamous carcinoma（39/74,52.70%）,but it was not found in tumor-adjacent mucosa（χ2=21.802,P〈0.01）.The expression of Twist was corrected with E-cadherin negatively（r=-0.433,P〈0.01）and N-cadherin positively（r=0.527,P〈0.01）in laryngeal squamous carcinoma.Overexpression of Twist and N-cadherin or down-regulated expressing of E-cadherin was correlated with higher pathologic grade,TNM clinic stage,thyroid cartilage involvement and lymph node metastasis.Follow-up study results showed that five-year overall survival rate for Twist positive group（39.3%）,that was higher than that for Twist negative group（21.7%）significantly（P〈0.01）.CONCLUSIONS Twist may be a crucial molecule in the regulation of EMT,and play important role in tumor＇s invasion and metastasis in laryngeal squamous carcinoma.Twist may be a valuable biomarker in evaluation of prognosis and a promising therapy gene target.
