摘 要:目的探讨多层螺旋计算机断层扫描(CT)后容积重建(VR)技术和表面遮盖显示(SSD)技术在肋骨转移瘤诊断中的临床应用价值。方法选取秦皇岛市第四医院2013年1月至2014年9月间收治的经手术及病理证实的75例肋骨转移瘤患者,所有患者均有多层螺旋CT检查资料,采集CT图像后分别采用VR及SSD技术进行图像处理,观察两种处理技术对肋骨转移瘤的诊断准确率。结果 75例肋骨转移瘤患者确诊发生成骨转移病灶148个、溶骨性转移病灶35个、混合型骨折病灶110个、病理骨折病灶27个、膨胀性生长病灶60个,VR诊断的准确率分别为99.3%、97.1%、98.2%、70.4%和98.3%,SSD诊断准确率分别为37.8%、74.3%、74.5%、96.3%和95.0%,两种技术对成骨转移病灶、溶骨性转移病灶、混合型骨折病灶、病理骨折病灶诊断的准确率间差异均有统计学意义(均P<0.05)。结论多层螺旋CT后VR技术在发现成骨转移病灶、溶骨性转移病灶、混合型骨折病灶方面具有显著优势,应作为首选的后处理技术,而SSD技术在诊断病理骨折病灶时具有一定的优势,可根据患者情况选择应用。Objective To explore the clinical application value of multi-slice spiral computed tomography( CT) post-processing volume renering( VR) and shaded surface display( SSD) in the diagnosis of costal metastasis. Methods From January 2013 to September 2014 in The Fourth Hospital of Qinghuangdao,75 cases of costal metastasis proved by operation and pathology were selected,and all patients had the image data of multi-slice spiral CT. After the CT images they used VR and SSD technology for image processing,and then the diagnostic accuracy of costal metastasis of two processing techniques were observed. Results 75 cases of costal metastasis diagnosed with bone metastases were148 cases,osteolytic metastases 35 cases,mixed fracture 110 cases,pathologic fractures 27 cases,expansive growth occurred cases,the diagnostic accuracy rate of VR were 99. 3%,97. 1%,98. 2%,70. 4%,98. 3% respectively,and the diagnostic accuracy rate of SSD were 37. 8%,74. 3%,74. 5%,96. 3%,95. 0%. The diagnostic accuracy rate of bone metastases,osteolytic metastases,mixed fracture and pathologic fractures of two techniques had significant differences( P〈0. 05). Conclusions Multi-slice spiral CT post-processing VR in the diagnosis of bone metastases,osteolytic metastases,mixed fracture costal metastasis has a significant advantage and it should be treated as the preferred technology. The SSD technology has certain advantages in the diagnosis of pathologic fractures,and the application should be selected based on the patient's condition.
