摘  要:目的探讨老年急性白血病(AL)患者染色体核型异常与FAB分型及预后的关系。方法 80例初治老年AL患者按照FAB分型标准分为急性髓细胞白血病(AML)49例和急性淋巴细胞白血病(ALL)31例;抽取患者骨髓标本,采用R显带技术分析染色体核型,分析染色体核型异常与FAB分型及预后的关系。结果 80例AL患者中染色体核型正常20例(25.0%),核型异常60例(75.0%);其中49例AML患者中染色体核型异常28例(57.1%),31例ALL患者中染色体核型异常12例(38.7%),AML患者染色体核型异常率显著高于ALL患者(χ2=6.783,P<0.01)。28例染色体核型异常AML患者总缓解率为85.7%,21例染色体核型正常AML患者总缓解率为90.5%,二者比较差异无统计学意义(χ2=0.253,P>0.05)。12例染色体核型异常ALL患者总缓解率为58.3%,19例染色体核型正常ALL患者总缓解率为89.5%,染色体核型正常ALL患者治疗总缓解率显著高于染色体核型异常患者(χ2=4.080,P<0.05)。结论特异性染色体异常可能是导致AL的细胞遗传学特征,且与FAB分型有关。通过染色体核型分析,可为化学治疗效果及预后评估提供参考。Objective To explore the relationship between abnormal chromosome karyotype and FAB classification or prognosis in elderly patients with acute leukemia( AL). Methods A total of 80 elderly patients with AL were divided into 49 cases of acute myelocytic leukemia( AML) and 31 cases of acute lymphocytic leukemia( ALL) according to FAB classification.Bone marrow sample of the patients was collected,the chromosome karyotypes were analyzed by R chromosome banding technique,then the relationship between abnormal chromosome karyotype and FAB classification or prognosis of the patients was analyzed. Results In 80 cases of AL,20 cases( 25. 0%) had normal chromosome karyotype,60 cases( 75. 0%) had abnormal chromosome karyotype. In 49 cases of AML,28 cases( 57. 1%) had abnormal chromosome karyotype. In 31 cases of ALL,12cases( 38. 7%) had abnormal chromosome karyotype. The rate of abnormal chromosome karyotype in patients with AML was significantly higher than that in patients with ALL( χ2= 6. 783,P〈0. 01). The total remission rate of 28 AML patients with abnormal chromosome karyotype was 85. 7%,the total remission rate of 21 AML patients with normal chromosome karyotype was90. 5%,the difference was not significant( χ2= 0. 253,P〉0. 05). The total remission rate of 12 ALL patients with abnormal chromosome karyotype was 58. 3%,the total remission rate of 19 AL patients with normal chromosome karyotype was 89. 5%,the total remission rate of the patients with normal chromosome karyotype was significantly higher than that of the patients with abnormal chromosome karyotype( χ2= 4. 080,P〈0. 05). Conclusion Specific chromosome abnormality may be the cytogenetical characteristic of AL,which is related to FAB classification. The analysis of chromosome karyotype can provide a reference for evaluating chemical treatment effect and prognosis.
