温州地区女性体检人群人乳头状瘤病毒感染及分布情况研究

摘　　要：目的了解温州地区女性体检人群宫颈脱落细胞HPV感染情况、型别分布及其特点。方法收集6 845例健康体检女性的宫颈脱落细胞标本,采用流式荧光杂交法检测26种HPV基因型,对HPV感染情况及其型别进行分析。结果6 845例患者中检出HPV感染者1 172例,感染阳性率为17.12%。HPV感染者中基因型的分布以高危型为主,居前3位的是HPV 52型、HPV 58型、HPV 16型,分别占感染人群的20.99%、12.29%、9.04%;低危型以HPV 61型、HPV44型、HPV 6型为主,分别占感染人群的8.53%、6.06%、1.88%。多重亚型感染占感染人群的16.55%,以HPV 52型和HPV 39型或HPV 52型和HPV 56型合并感染者居多。在多重感染人群中,随着合并感染亚型数的增加,人群占比逐渐降低。>65岁年龄组的HPV感染率高达24.21%,明显高于其他年龄组,差异有统计学意义(P<0.001)。结论温州地区女性健康体检人群HPV感染常见亚型为HPV 52型、HPV 58型、HPV 16型,>65岁年龄段HPV感染率最高,相关部门应加强此类人群HPV的预防控制。Objective To investigate the exfoliative cell HPV infection situation as well as the genotype distribution and characteristics from physical examination women in Wenzhou. Methods To collect 6 845 cases of cervical exfoliated cells specimens from healthy women,and to use flowcytometry fluorescence hybridization method to detect 26 genetypes of HPV. The HPV infection situation and their genotypes were analyzed. Results 1 172 cases with HPV infection were detected from the 6 845 women patients and the positive infection rate was 17. 12%. HPV positive patients with high- risk genotypes took the largest proportion. The top 3 were HPV 52,HPV 58,HPV 16,respectively accounting for 20. 99%,12. 29%,9. 04%. The low-risk genotypes were mainly HPV 61,HPV 44 and HPV 6,respectively accounting for 8. 53%,6. 06% and 1. 88%. Multiple subtypes infection accounted for 16. 55% of the infected patients,HPV 52 combined with HPV 39 and HPV 52 combined with HPV 56 accounted for the largest proportion. In the multiple infected patients,with the increase of the number of patients infected with subtypes,its proportion reduced gradually. The HPV infection rate of the above 65 years old group was 24. 21%,which was higher than the other age groups,and the difference had statistical significance（ P〈0. 001）. Conclusion HPV infection subtypes among the women who have taken physical examination in Wenzhou are mainly HPV 52,HPV 58 and HPV 16,the highest infection rate is with the above 65 years old group,and the relevant departments should strengthen the prevention and control of HPV among this kind of people.
