口轮匝肌重叠缝合在单侧唇裂人中重建中的应用

摘　　要：目的探讨口轮匝肌重叠缝合在单侧唇裂患儿人中重建中的疗效。方法 2012年2月—2013年3月,采用口轮匝肌重叠缝合的方法,对50例单侧唇裂患儿进行人中结构的重建,其中男性33例,女性17例（平均年龄6.5个月）。使用3阶评分法对术后患儿人中嵴高度进行评价。结果术后切口均一期愈合,随访3~12个月,48例人中嵴及人中凹结构自然,人中嵴高度与健侧接近,外形满意,评分1~2分。2例患儿出现较平坦的人中嵴。结论对于单侧唇裂患儿人中结构的重建,口轮匝肌重叠缝合是一种简单有效的治疗方法。Objective To investigate a method to reconstruct the philtrum of unilateral cleft lip with vertical interdigitation of orbicularis oris muscle flap. Methods Between February 2012 and March 2013,50 patients,including 33 male and 17 female patients（ mean age,6. 5months）,underwent unilateral cleft lip repair. The philtrum was reconstructed by the vertical interdigitation of orbicularis oris muscle flap method. The aesthetic outcome of the philtrum was scored to evaluate the effectiveness of the procedure. Results All the cases were followed up 3 months to 12 months after operation. Good symmetry of the bilateral philtral ridges was achieved in 48 of the 50 patients,with the postoperative scores for aesthetic outcome being 1 ~ 2. Only 2 patients showed less prominent philtral ridges on the reconstructed side than on the normal side. Conclusion The technique of vertical interdigitation of orbicularis oris muscle flap showed good effect in patients with unilateral cleft lip.
