海洛因依赖者强制戒毒环境中生存质量的相关因素分析

摘　　要：目的:了解海洛因依赖者生存质量(QOL)的相关因素。方法:收集201例强制隔离戒毒所海洛因依赖者的吸毒特征,并进行医学结局研究简明健康调查量表(SF-36)、流调中心用抑郁量表、简易应对方式问卷和生活事件量表测评,用结构方程模型分析各因素对SF-36量表的生理和心理QOL的作用路径及效应大小。结果:通径分析显示,生理QOL的直接预测因素(β,标化通径系数)有已婚(0.13)、文化程度高(0.27)、年龄大(-0.18)、吸毒年限长(-0.11)和抑郁程度重(-0.46),心理QOL的直接预测因素(β)有消极应对(-0.19)和抑郁程度重(-0.58);注射吸毒和负性生活事件分高(β)均通过抑郁间接预测生理QOL(-0.09和-0.10)和心理QOL(-0.12和-0.09),负性生活事件分高和年龄大通过消极应对间接预测心理QOL(-0.04和-0.04)。结论:海洛因依赖者的QOL受社会、心理和吸毒因素的影响,综合性的心理社会干预措施可改善海洛因依赖者的QOL。Objective： To investigate the correlates of quality of life （QOL） among heroin-dependent pa- tients. Method：A total of 201 heroin addicts were administered with medical outcomes study 36-item short- form health survey （SF-36）, center for epidemiological studies depression scale, simplified coping style ques- tionnaire and life event scale. We also investigated patients＇ drug use characteristics. Structured equation model was used to analyze influencing pathways and effects of factors on physical and mental QOL of SF-36. Results：Path analysis showed that, the direct predictors （fl, standardized path-coefficient） of physical QOL were being married （ 0. 13 ）, high level of education （ 0. 27 ）, older age （ - 0. 18 ）, long duration of drug use （ -0.11 ） and high level of depression severity （-0.46） ,while the direct predictors （fl） of mental QOL were negative coping style （ - 0. 19 ） and high level of depression severity （ - 0. 58 ）. Injecting drug use and high score of negative life event （/3） indirectly predicted physical QOL （ -0. 09 and -0. 10） and mental QOL （ -0. 12and -0. 09） through depression,while high score of negative life event and older age （fl） indirectly predicted mental QOL （ - 0. 04 and - 0. 04） through negative coping style. Conclusion： QOL of heroin ad- dicts is influenced by multiple factors in social, psychological and drug use domains. Comprehensive socio-psy- chological interventions may be effective for improving the QOL of heroin addicts.
