GP73、AFP-L3与GPC-3联合检测在原发性肝癌诊断中的临床价值

摘　　要：目的探讨高尔基体蛋白73(GP73)、甲胎蛋白异质体3(AFP-L3)和磷脂酰肌醇蛋白聚糖-3(GPC-3)联合检测在原发性肝癌(PHC)诊断中的临床价值。方法采用免疫组织化学法和酶联免疫法检测原发性肝癌(PHC组,n=34例)、肝硬化(LC组,n=25例)和病毒性肝炎(VH组,n=27例)患者肝组织和血清GP73、AFP-L3和GPC-3的表达水平,同期选择30例门诊正常体检者作为对照组。观察各组肝组织和血清GP73、AFP-L3和GPC-3的表达水平,并比较GP73、AFP-L3和GPC-3单一和联合检测PHC的阳性率。结果PHC组肝组织及血清AFP-L3、GP73和GPC-3表达水平均明显高于其他各组(P<0.05),3项标志物(AFP-L3、GP73和GPC-3)单项检测PHC的阳性率分别为70.6%、76.5%、79.4%,两两指标联合检测PHC的阳性率可至82.3%、85.3%和88.2%,3项指标联合检测PHC的阳性率可高达91.2%,明显高于单项检测率(P<0.05)。结论 AFP-L3、GP73和GPC-3联合检测可提高PHC阳性率,对早期PHC诊断具有重要的临床意义。Objective To investigate the clinical value of Golgi protein 73（GP73）,alpha fetoprotein heterogeneity 3（AFP-L3）combined Glypican-3（GPC-3）detection with in the diagnosis of primary hepatic carcinoma.Methods The expression levels of GP73,AFP-L3 and GPC-3in liver tissue and serum of primary hepatic carcinoma（group PHC,n=34）,liver cirrhosis（group LC,n=25）and viral hepatitis（group VH,n=27）patients were detected by immunohistochemical and enzyme immunoassay.At the same period,30 cases of normal examination were selected as control group.The expression levels of GP73,AFP-L3 and GPC-3in liver tissue and serum of all groups were observed,and compared the positive rate of GP73,AFP-L3,GPC-3single detection and combined detection to primary hepatic carcinoma.Results The expression levels of GP73,AFP-L3 and GPC-3in the liver tissue and serum of PHC group were significantly higher than that of other groups（P〈0.05）.The single test positive rate of AFP-L3,GP73 and GPC-3were 70.6%,76.5%,79.4%to PHC,two indicators combined detection of PHC were 82.3%,85.3% and 88.2%,3indicators combined detection of PHC was 91.2%.The rate of 3indicators combined detection was higher than that of single indicator（P〈0.05）.Conclusion The combined detection of AFP-L3,GP73 and GPC-3could improve the positive rate of PHC.It had an important clinical significance for early diagnosis of PHC.
