海绵窦海绵状血管瘤CT、MRI表现分析

摘　　要：目的探讨海绵窦海绵状血管瘤CT及MRI特征。方法回顾性分析6例经手术病理证实的海绵窦海绵状血管瘤患者的CT与MRI资料。6例均行MRI平扫+增强扫描及DWI扫描,1例行MRA检查;3例并行CT平扫+增强及CTA检查。结果肿瘤发生于右侧海绵窦3例,左侧3例。5例呈葫芦样横跨海绵窦,MR T1WI呈低信号;3例见小条片状等稍高信号,T2WI呈高信号,2例见小条状低信号,MR DWI上呈等或偏低偏高信号,ADC图呈低信号,增强扫描显著填充式强化。3例行CT检查,平扫呈均匀稍高密度,CTA或MRA可见病变包绕颈内动脉血管,病变未见明显供血血管;1例T1WI、T2WI像上呈混杂信号,CT可见明显钙化,增强扫描非钙化区域呈填充式强化特征。结论海绵窦海绵状血管瘤CT及MRI平扫及增强扫描影像表现具有特征性,认识这些特征并结合DWI及血管检查,有助于正确诊断。Objective To investigate the CT and MR features of cavernous hemangioma in cavernous sinus,in order to improve the accuracy rate of preoperative diagnosis. Methods A retrospective analysis of 6 cases of cavernous hemangioma confirmed by operation and pathology in cavernous sinus（ 1 male and 5 female）,all underwent MR plain scan,contrast enhanced scan and DWI,MRA was performed in 1 case,and 3 cases with contrast enhanced CT and CTA examination.Results Among the 6 cases,there were 3 cases that the tumor in the right cavernous sinus and other 3 were in the left; In5 cases,the tumor was gourd-like across the cavernous sinus; MR T1 WI showed hypointensity in 5 cases,of which 3 cases showed plaque-like slightly hyperintensity and hyperintensity on T2 WI,while 2 cases showed plaque-like hypointensity on T2 WI,DWI showed iso-,hypo-or hyperintensity,the apparent diffusion map showed hypointensity,contrast enhanced MR scan showed significantly filling type contrast enhancement; Among them,3 cases were underwent CT scan. plain CT scan showed homogeneous slightly high density,CTA or MRA showed the lesion wrap around the internal carotid artery,but there was no obvious blood-supply vessels for the lesion. 1 cases showed mixed signal intensity on T1 WI and T2 WI while CT showed obvious calcification in the lesion,and contrast enhanced CT scan showed filling type enhancement of the non calcified area. Conclusion There are characteristic imaging findings on CT and MR scan of cavernous hemangioma in cavernous sinus,it is helpful for the correct diagnosis to understanding these characteristics combined with MR DWI and vascular examination（ MRA or CTA）.
