飞秒激光与机械刀制瓣LASIK术后角膜后表面曲率变化临床观察  被引量:8

摘  要:目的对飞秒激光辅助准分子激光原位角膜磨镶术(FSLASIK)、前弹力层下准分子激光角膜磨镶术(SBK)、机械板层刀制瓣LASIK(传统LAISK)术后角膜后表面曲率的变化进行临床观察。方法临床病例对照研究。选择2013年9月至2014年3月在沈阳爱尔眼视光医院近视及近视散光患者192例(384只眼),分为FSLASIK组、SBK组及传统LAISK组,每组患者64例(128只眼),分别在术前、术后1周、1、3个月、半年及1年使用Pentacam眼前节分析系统测量角膜后表面曲率,术前角膜后表面曲率值FSLASIK组-5.70--6.70D,平均(-6.267±0.206)D;传统LAISK组-5.70--6.70D,平均(-6.282--0.188)D:SBK组-5.70—-6.80D,平均(-6.283±0.214)D。结果FSLASIK和SBK术后角膜后表面曲率变化较小,两种手术方式在术后不同时间角膜后表面曲率的差异无统计学意义(P〉0.05);传统LAISK术后1周与术前比较角膜后表面曲率变化差异无统计学意义(P〉0.05),术后1个月到1年角膜后表面曲率与术前相比有明显变化,差异有统计学意义(P〈0.05)。结论FSLASIK及SBK术后各时间角膜后表面曲率未发生明显变化,同传统LAISK相比对角膜形态影响更小、安全性更高。Objective To analyze the changes of curvature of posterior corneal surface after LASIK with Femtosecond (FSLASIK), Sub-Bowman's keratomilesis (SBK), LASIK with mechanical microkeratomc (Traditional LASIK) on myopia. Methods Choose 192 myopia and myopic astigmatism patients (384 eyes) to Shenyang eye Hospital Department of ophthalmology, divided into FSLA- SIK group, SBK group and Traditional LAISK group, 64 cases of patients in each group (128 eyes). The curvature of posterior corneal surface was measured by Petancam before and after operation (1 week, 1 month, 3 months, 6 months and 1 year). The preoperative curvature of posterior corneal surface was-6.267 ± 0.206 D (ranged from -5.70 to-6.70 D), -6.282 ± 0.188 D (ranged from-5.70 to -6.70 D),-6.283 ± 0.214 D (ranged from-5.70 to-6.80 D) in FSLIASK, Traditional LASIK, SBK group. Results There was no statistically significant difference in the changes of curvature of posterior corneal surface between FSLASIK and SBK at different time points. There was no statistically significant difference in the changes of curvature of posterior corneal surface between preoperative and 1 week postoperatively in traditional LAISK. There was statistically significant difference in the changes of curvature of posterior corneal surface between preoperative and 1 month, 1 year postoperatively in traditional LAISK. Conclusions There is no significant change at different time points after operation in FSLASIK and SBK. Compared with traditional LAISK, FSLASIK and SBK have smaller influence on the cornea and higher security.
