摘 要:目的探讨多次染色在治疗伴黄斑前膜脉络膜脱离型黄斑裂孔性视网膜脱离的疗效及并发症。方法总结2006年1月至2013年2月在唐山市眼科医院治疗的伴黄斑前膜脉络膜脱离型黄斑裂孔性视网膜脱离患者79例(79眼)。手术均采用曲安奈德标记、亮蓝染色辅助下玻璃体切割,黄斑前膜及内界膜剥离,硅油填充术式。在3个月行硅油取出C3F8气体填充术。术后随访6~24个月,观察随访末期视网膜复位、脉络膜复位、视力变化及并发症情况。结果黄斑裂孔封闭视网膜复位率96.2%(76/79);脉络膜全部复位;术中视网膜有少量出血;术后视力有不同程度改善;OCT检查未见术后黄斑前膜复发;术后一过性眼压升高,后来眼压恢复正常;术后炎症反应轻。结论多次染色在治疗伴黄斑前膜脉络膜脱离型黄斑裂孔性视网膜脱离取得良好疗效,不同程度上改善了视力,减少了并发症发生。Objective To evaluate the effects and complication of multiple staining for macular eptiretinal membrane and choroidal detachment with macular hole retinal detachment. Methods Retrospectively analyzed 79 cases (79 eyes) with macular epiretinal membrane and choroidal detachment associated with macular hole retinal detachment from January 2006 to February 2013. All the cases underwent the application of marking by Triamcindone acetonide (TA) and staining by brilliant blue G (BBG), vitrectomy, macular epiretinal membrane and internal limiting membrane peeling combined with silicon oil filling operation treatment. Silicone oil tamponade in 79 eyes underwent removal at an approximate interval of 3 months, and all the eyes were temponaded with C3F8. The follow-up period ranged from 6 months to 24 months, the retinal reposition, the final visual acuity changes and complications were observed. Results The rate of macular hole closure and retinal reposition was 96.2% (76/79), eyesight all the cases were improved in different extents. The complications included the slight retinal hemorrhage and secondary glaucoma or postoperative transient high intmocular pressure, and then became stable. The inflammatory reaction was mild. Optical coherence tomography (OCT) ex- amination, secondary epimacular membrane formation was not found. Conclusions Multiple staining is very effective for macular epiretinal membrane and choroidal detachment with macular hole retinal detachment. It could improve vision in different extents and decrease the occurrence of complication.
