低位直肠癌保肛根治术32例应用体会

摘　　要：目的总结保留盆腔自主神经的全直肠系膜切除术在低位直肠癌保肛手术中的应用体会。方法回顾分析应用保留盆腔自主神经的全直肠系膜切除术结合双吻合器技术治疗的32例低位直肠癌患者的临床资料。结果术后局部复发率为6.25%,5年生存率为75.0%,排尿功能障碍12.5%,性功能障碍18.75%。结论低位直肠癌应用保留盆腔自主神经的全直肠系膜切除术可有效降低局部复发率和提高生存率,并能较好保留直肠癌患者术后的排尿和性功能,是有效、安全的理想术式。Objective Pelvic autonomic nerve preservation discussed the total mesorectum excision of ap-plication in low rectal cancer anal surgery. Methods Retrospective analysis of pelvic autonomic nerve preserva-tion application of total mesorectal excision combined with double stapling technique for the treatment of the clin-ical data of 32 patients with low rectal cancer. Results Local recurrence rate was 6. 25%, and 5-year survival rate was 75%, 12. 5% voiding dysfunction, sexual dysfunction （18. 75%）. Conclusions Low rectal cancer pelvic autonomic nerve preservation application of total mesorectum excision can effectively reduce the local re-currence rate and improve survival rate, and can better keep colorectal cancer patients with postoperative mictu-rition and sexual function. Sphincter preservation in total mesoretal excision of low rectal carcinoma is effective, safe and relatively ideal operating style.
