儿童青少年注意缺陷多动障碍合并品行障碍的调查研究

摘　　要：目的探讨儿童青少年注意缺陷多动障碍（ADHD）合并品行障碍（CD）的流行特征。方法采用多阶段随机抽样方法,参照美国精神障碍诊断和统计工作手册第四版的注意力缺陷多功能障碍诊断标准和DSM-Ⅳ标准编写调查表,对2046例4~16岁儿童青少年进行调查。结果本次研究ADHD合并CD学生总患病率为3.86%,其中男性高于女性（P〈0.05）混合型ADHD合并CD男女患病率差异比较无统计学意义（P〉0.05）;ADHD男性合并CD的患病率高于无ADHD男性的（P〈0.05）;ADHD女性合并CD的患病率高于非ADHD女性（P〈0.05）;随年龄的增加,注意缺陷、多动-冲动为主型和混合型ADHD合并CD患病率呈下降趋势（P〈0.05）;不同ADHD亚型合并CD、非ADHD合并CD的照顾者构成情况差异无统计学意义（P〉0.05）。结论年龄和性别对儿童青少年注意力缺陷障碍合并品行障碍的发生有一定的影响,而儿童青少年生活照顾者对患病率没有明显影响。Objective To investigate children and adolescents with attention deficit hyperactivity disorder combined with conduct disorder epidemic characteristics. Methods Used multi stage random sampling method, re- ferring to the United States mental disorders Diagnostic and Statistical Manual fourth edition attention deficit multi- function diagnostic criteria and DSM-1V standard written questionnaire, investigated 2046 cases of 4 ~ 16 years old children and adolescents. Results The study in the ADHD merger CD students overall prevalence was 3.86%, of which men than women （P〈0. 05） hybrid combined CD and prevalence of ADHD diff without statistical significance （P〉0. 05） ; The prevalence of ADHD men combined CD higher than that of without ADHD who （ P 〈 0. 05 ） ADHD women merged the prevalence of CD is higher than the ADHD （P〈0. 05） ; With the increase of age, atten- tion deficit, hyperactivity - impulsivity as main type and Hybrid ADHD combined with CD prevalence rate as- sumed the drop tendency （P〈0. 05） ; Different ADHD subtypes combined with CD and non ADHD combined with CD caregivers composition difference was no statistical significan （P〉0. 05）. Conclusion Age and sex have cer- tain influence on the occurrence of children and adolescents with attention deficit hyperactivity disorder combined with conduct disorder, and child and adolescent Life care givers had no significant influence on the prevalence.
