眼铁质沉着症临床表现及手术疗效的研究

摘　　要：目的 探讨眼铁质沉着症的临床特点、治疗方案及预后。方法 对32例眼铁沉着症患者进行回顾性分析。结果 异物存留于眼内15天-20年。手术前最佳矫正视力〈0.05,0.05-0.3和〉0.3者分别为16、12和4眼。常规后径摘出3例,联合小梁切除术2例;经玻璃体的眼内异物摘出27例,其中24例联合晶状体摘除术;经晶状体摘除取异物联合人工晶状体植入术2例。30例患者术后最佳矫正视力得到提高,手术后〈0.05,0.05-0.3和〉0.3者分别为4、7和21只眼,最高从术前光感提高至术后1.0。手术后最佳矫正视力〈0.05者,异物存留于眼内时间均超过5年。结论 眼铁质沉着症手术治疗安全、有效,明显改善患者视功能。异物存留于眼内时间过长者术后视功能恢复较差。Objective To explore the clinical feature,treatment and prognosis of ocular siderosis. Methods 32 patients （32 eyes） with ocular siderosis were retrospective reviewed. Results The interval time between injury and foreign body was 15 days to 20 years. The best corrected visual acuity （BCVA） of 16 eyes was less than 0.05,12 eyes was between 0.05 and 0.3 and 4 eyes was higher than 0.3. Foreign body extraction surgery was performed from conventional back path in 3 eyes （2 eyes combined with trabeculectomy）,from vitreous in 27 eyes（24 eyes combined with lens extraction） and from lens combined with intraocluar lens implatation in 2 eyes. Improved BCVA after surgery was obtained in 30 eyes with a best outcome from light perception to 1.0. BCVA〉0.3 postoperative in 21 eyes（65.6%）,0.05-0.3 in 7 eyes（21.9%）,〈0.05 in 4 eyes（12.5%）with foreign body stagnation for more than 5 years. Conclusion Surgery was a safe and effective treatment approach with significant improve of visual acuity to ocular siderosis. Long stagnation of foreign body lead to poor prognosis.
