基层医院晶状体超声乳化术术后并发症的分析

摘　　要：目的：分析基层医院晶状体超声乳化人工晶状体植入术术后并发症的发生情况，探讨其发生原因和对策。方法回顾性分析2010年1月至2014年8月间在本院实施白内障的晶状体超声乳化人工晶状体植入手术1561例（1968眼）的临床资料，对其术后并发症进行分析和探讨。结果1968眼超声乳化人工晶状体植入术后发生角膜水肿345眼、一过性高眼压232眼、葡萄膜炎反应219眼、屈光不正74眼、干眼76眼及后发性白内障109眼。结论白内障术后主要并发症是角膜水肿和葡萄膜炎反应，合理采取防治措施，可有效避免或减轻并发症，避免视功能的损害，避免医疗纠纷的发生。只要准备充分，开展院院合作，在基层医院开展白内障的超声乳化人工晶状体植入术安全可靠；为基层群众提供高质量的医疗服务。Objective To analyse the complications after phacoemulsification and intraocular lens （ IOL） implantation and to investigate the cause and the countermeasures in primary hospitals. Methods The clinlical data of 1968 eyes of 1561 cataract patients who underwent phacoemulsification and IOL implantation in our hospital from January 2010 to August 2014 were retrospectively analyzed. The complications were analyzed and discussed. Results Among the 1968 eyes of phacoemulsification and IOL implantation, postoperative corneal edema occurred in 345 eyes, transient high intraocular pressure occurred in 232 eyes, uveitis reaction occurred in 219 eyes, refractive error occurred in 74 eyes, postoperative dry eye symptoms occurred in 76 eyes, and posterior capsular opacification occurred in 109 eyes. Conclusion The main complications after cataract surgery were corneal edema and postoperative uveitis. Taking reasonable measures can effectively prevent the occurrence of complications, and can avoid the visual function impairment and the medical disputes. As long as making adequate preparations and carrying out the cooperation between hospitals, phacoemulsification and intraocular lens implantation for cataract in primary hospitals is safe and reliable with high quality health services.
