摘 要:目的观察宫颈正常细胞、宫颈上皮内瘤变(CIN)Ⅲ级病变和鳞状细胞癌组织标本中人乳头瘤病毒(HPV)感染的基因型分布情况及其临床意义。方法采用PCR和基因芯片检测技术对1 047例宫颈正常细胞、319例宫颈CINⅢ级病变和248例宫颈鳞状细胞癌组织标本进行23种HPV基因分型检测,并对受检者相关资料进行分析。结果 1 047例宫颈细胞正常女性人群检出HPV感染者109例,HPV总感染率为10.41%;319例宫颈CINⅢ级病变患者检出HPV感染者300例,HPV总感染率为94.04%;248例宫颈鳞状细胞癌患者检出HPV感染者223例,HPV总感染率为89.92%。结论 PCR与基因芯片检测技术可应用于宫颈细胞和组织标本,一次可检测23种HPV基因型别,特异性强,敏感性高,对我国女性宫颈HPV感染的分子流行病学调查、宫颈癌前病变、鳞状细胞癌的防治及其疫苗的研究具有非常重要的意义。Objective To observe the distribution of human papillomavirus (HPV) infection genotypes in normal cervical cells, cervical intraepithelial neoplasia (CIN)Ⅲ, and cervical squamous cell carcinoma and the clinical significance. Methods PCR and gene chip technology were used to detect 23 kinds of HPV genotypes in cervical specimens of 1 047 cases with normal cervical cells, 319 cases with CIN Ⅲ, and 248 cases with cervical squamous cell carcinoma; the related data was analyzed. Results Among 1 047 women with normal cervical cells, 109 women were diagnosed as HPV infection, the total infection rate of HPV was 10. 41% ; among 319 eases with CIN Ⅲ, 300 women were diagnosed as HPV infection, the total infection rate of HPV was 94.04% ; among 248 cases with cervical squamous cell carcinoma, 223 women were diagnosed as HPV infection, the total infection rate of HPV was 89. 92%. Conclusion PCR and gene chip technology can be used for cervical cells and tissues, the two methods can detect 23 kinds of HPV genotypes at the same time with strong specificity and high sensitivity, which have great clinical significance for molecular epidemiological investigation of cervical HPV infection, prevention and treatment of cervical precancerous lesion, and cervical squamous cell carcinoma and development of HPV vaccine.
