优质护理干预对降低鼻咽癌调强放疗后口腔黏膜反应的影响

摘　　要：目的:探讨优质护理干预对降低鼻咽癌调强放疗后口腔黏膜反应的影响。方法:早期鼻咽癌患者80例根据随机抽签原则分为治疗组与对照组各40例,所有患者都选择调强放疗,在放疗期间对照组给予传统护理,治疗组在此基础上加用积极地优质护理干预,干预周期为6周。结果:干预后治疗组的生理功能、心理精神、社会关系、治疗评分明显高于对照组(P<0.05)。干预后治疗组与对照组的有效率分别为55.0%和45.0%,治疗组放疗有效率较对照组高,将其进行对比,其差异在统计学上没有意义(P>0.05)。治疗组在进行干预期间的贫血、血小板减少、食欲下降等不良反应发生情况明显少于对照组(P<0.05),两组脱发发生率无差异。干预后治疗组的口腔黏膜反应发生例数与程度都明显少于对照组(P<0.05)。结论:调强放疗在鼻咽癌治疗中的应用有很好的放疗效果,而优质护理干预的应用能减少口腔黏膜反应与其他不良反应的发生,从而提高患者放疗后生活质量。Objective： To investigate the reducing oral mucositis effects after IMRT radiotherapy of quality care interventions for the nasopharyngeal patients. Methods： All 80 patients with early stage nasopharyngeal carcinoma were equally divided into the treatment group and the control group. All patients were given the radiotherapy IMRT,during radiotherapy,the control group were given conventional treatment,on the basis,the treatment group were added the quality care intervention. Results： The physiological functions,psychological spiritual,social relations,treatment scores in the treatment group after intervention were significantly higher（ P 0. 05）. After the intervention,the response rates in the treatment group and the control group were 55. 0% and 45. 0% respectively,but compared no significant difference（ P 0. 05）. During intervention,anemia,thrombocytopenia,alopecia,loss of appetite and other adverse effects in the treatment group were significantly less than in the control group（ P 0. 05）. The number of cases of oral mucositis reaction in the treatment group were significantly less than the control group（ P 0. 05）. Conclusion： IMRT radiotherapy in nasopharyngeal carcinoma therapy has good effect,and the application of quality care interventions can reduce the occurrence of oral mucositis and other side effects,improve the quality of life of patients after radiotherapy.
