左房黏液瘤临床特征及经胸超声心动图对其的诊断价值

摘　　要：目的探讨左房黏液瘤的临床特征及经胸超声心动图对其的诊断价值。方法 65例经手术病理确诊的左房黏液瘤患者,应用经胸超声心动图测量瘤体最大径、左房内径、二尖瓣反流、三尖瓣反流、肺动脉收缩压(PASP)、左室射血分数(LVEF)及二尖瓣口面积(MVA)。根据MVA测值将患者分为>2 cm2组44例(A组)和≤2 cm2组21例(B组),比较两组患者一般临床特征和超声心动图指标差异,分析瘤体最大径与PASP及MVA间的相关性。结果两组患者性别、年龄、胸部不适症状比例及LVEF比较差异均无统计学意义,两组患者栓塞症状比例、瘤体最大径、左房内径、二尖瓣反流、三尖瓣反流及PASP比较差异均有统计学意义(P<0.05)。58例患者瘤体最大径与PASP呈正相关(r=0.62,P<0.01);A组患者瘤体最大径与MVA呈负相关(r=-0.70,P<0.01)。结论左房黏液瘤瘤体最大径与PASP及MVA相关;经胸超声心动图对左房黏液瘤的诊断有重要价值。Objective To analyze the clinical features and the diagnostic value of transthoracic echocardiography（TTE） for left atrial myxoma. Methods Sixty- five patients with left atrial myxoma confirmed by operation were enrolled. The maximum diameter of the myxoma（MD）,left atrial diameter（LAD）,mitral regurgitation（MR）,tricuspid regurgitation（TR）,pulmonary artery systolic pressure（PASP）,left ventricular ejection fraction（LVEF） and mitral valvular area（MVA） were measured by TTE.The patients were divided into2 cm2 group（group A,n=44） and ≤2 cm2group（group B,n = 21） according to MVA. The clinical features and indexes of TTE between the two groups were compared.Correlation between MD and PASP,MVA was analyzed.Results Differences of gender,age,clinical features and LVEF between two groups were not significant.Significant differences between two groups were found in embolic events,MD,LAD,MR,TR and PASP（P〈0.05）.MD was positively correlated with PASP（r=0.62,P〈0.01） in 58 patients.MD was negatively correlated with MVA in group A（r=-0.70,P〈0.01）.Conclusion Myxoma MD showed correlation with PASP and MVA.TTE is an essential assessing method for diagnosis of left atrial myxoma.
