摘  要:目的探讨超声弹性成像在诊断乳腺BI-RADS评分3级及4级乳腺病变中的价值。方法以病理结果为金标准,根据超声弹性成像结果对149例BI-RADS评分3级及4级的乳腺病变进行校正,比较校正前后的诊断结果。结果应变率比值法诊断乳腺肿块的ROC曲线下面积为0.920,弹性评分法和应变率比值法诊断乳腺恶性肿块的敏感性、特异性、阳性预测率、阴性预测率、诊断准确率分别为77.36%,87.50%,77.36%,87.50%,83.89%和69.81%、97.91%、94.87%、85.45%、87.9%。弹性评分法和应变率比值法校正后的BI-RADS评分分级诊断乳腺恶性肿块敏感性、特异性、阳性预测率、阴性预测率及诊断准确率分别为83.01%、97.91%、95.65%、91.23%及92.62%,与校正前比较差异有统计学意义(P<0.05)。结论超声弹性成像可以明显提高BI-RADS评分3级及4级乳腺病变诊断的准确率。Objective To explore the value of ultrasonic elastography(UE) in diagnosis of BI- RADS classification 3 and 4 breast lesions.Methods The BI-RADS classification of 149 lesions was corrected according to UE results,the comparison between BI-RADS classification and corrected BI-RADS classification was applied based on pathologic analysis.Results The area under the ROC curve of strain rate ratio in diagnosing breast lesions was 0.920.The sensitivity,specificity,positive predictive rate,negative predictive rate and accuracy rate of elastic scoring and strain rate ratio in diagnosing breast lesions were 77.36%,87.50%,77.36%,87.50%,83.89% and 69.81%,97.91%,94.87%,85.45%,87.9%,respectively.The sensitivity,specificity,positive predictive rate,negative predictive rate and accuracy rate of BI-RADS classification which was corrected by elastic scoring and strain rate ratio in diagnosing breast lesions were83.01%,97.91%,95.65%,91.23% and 92.62%,respectively,which had a significant difference with uncorrected BI-RADS classification(P〈0.05).Conclusion UE can improve the diagnostic accuracy for BI-RADS classification 3 and 4 breast lesions.
