摘　　要：目的观察神经节苷脂对超急性期脑梗死患者脑缺血再灌注损伤的保护作用。方法选择超急性期脑梗死患者72例,采用随机数字表法将患者分为对照组和观察组:对照组给予溶栓、脱水、抗凝、护脑、高压氧、对症治疗等常规治疗方法,观察组在对照组治疗的基础上应用神经节苷脂,治疗14 d。检测治疗前后的血清丙二醛(MDA)、血清超氧化物歧化酶(SOD)、基质金属蛋白酶-9(MMP-9)、相对表观扩散系数(r ADC)值及美国国立卫生研究院卒中量表评分(NIHSS),并评定临床疗效。结果治疗14 d后观察组患者的基本痊愈率及总有效率均高于对照组(P<0.05);两组患者的MDA、SOD、MMP-9、r ADC值及NIHSS评分均较治疗前改善(P<0.01),观察组患者较对照组改善更显著(均P<0.01)。结论神经节苷脂对超急性期脑梗死患者脑缺血再灌注损伤具有保护作用,该作用与减少氧自由基的产生有关。Objective To observe the protection effect of gangliosides on cerebral ischemia-reperfusion injury（CIRI） in patients with hyperacute cerebral infarction（HCI）. Methods 72 patients with HCI were randomly divided into two groups： the control group（36 cases） were treated with thrombolysis, dehydration, anticoagulation, brain protecting, hyperbaric oxygen and symptomatic treatment, the observation group（36 cases） were given the ganglioside based on the treatment in control group, two groups had been treated and observed for14 days. The levels of malondialdehyde（MDA）, superoxide dismutase（SOD）, matrix metalloproteinase 9（MMP-9）, relative apparent diffusion coefficient（rADC） and national institute of health stroke scale（NIHSS）scores were monitor before and after treatment,and the clinical efficacy was evaluated. Results After 14 days of treatment, the basic recovery rate and the total effective rate of observation group were higher than those in control group（P〈0.05）; MDA, SOD, MMP-9, rADC value and NIHSS score in two groups were improved after treatment（P〈 0.01）. Compared to the control group, MDA, SOD, MMP- 9, rADC value and NIHSS score were significantly improved in the observation group（P 〈0.01）. Conclusion Gangliosides has protective effect on CIRI caused by HCI, potentially related with reduced oxygen free radicals.
