摘 要:目的研究乳腺癌动态增强磁共振成像(DCE-MRI)表现与分子生物学指标雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(Her-2)、Ki-67表达的相关性。方法对53例乳腺癌患者共计55个肿块术前行DCE-MRI检查,分析其影像学表现(肿块大小、形态、边缘、时间信号强度曲线、强化形式、早期增强率)。术后标本行免疫组织化学染色测定肿瘤细胞ER、PR、Her-2、Ki-67表达情况。采用Spearman等级相关检验分析分子生物学指标与DCE-MRI表现的相关性。结果肿块大小与Ki-67阳性表达呈正相关;肿块形态与Her-2阳性表达呈正相关,与Ki-67阳性表达呈正相关;肿块边缘与ER、PR阳性表达呈正相关;肿块强化形式与PR阳性表达呈负相关;TIC类型与Ki-67阳性表达呈正相关;早期增强率与Her-2阳性表达呈正相关。结论乳腺癌DCE-MRI表现与分子生物学指标间存在一定的相关性,DCE-MRI表现可以反映肿瘤的生物学行为,能够间接评估肿瘤预后并为指导临床治疗方案的选择提供参考。Objective To study the correlation between features of dynamic contrast-enhanced magnetic reso- nance imaging (DCE-MRI) and the expression of molecular biological markers ER, PR, Her-2 and Ki-67 in breast cancer. Methods LCE-MRI was performed in 53 cases of breast cancer with 55 lesions before operation, and imag- ing features of breast cancer were analyzed. The expression of ER, PR, Her-2 and Ki-67 in postoperative specimens were measured by immunohistochemistry. Correlation between DCE-MRI features and molecular biology markers in breast cancer were analyzed using Spearman rank correlation test. Results Tumor size was positively correlated with Ki-67 positive expression; tumor morphology was positively correlated with Her-2 positive expression, and significantly positively correlated with Ki-67 positive expression; tumor edge was positively correlated with ER, PR positive expres- sion; Tumor enhancement pattern was negatively correlated with PR positive expression; time signal intensity curve (TIC) type was significantly positively correlated with Ki-67 positive expression; early enhancement rate was positive- ly correlated with Her-2 positive expression. Conclusion There is some correlation between the DCE-MRI features and molecular biology markers in breast cancer. DCE-MRI features can reflect the biologic behavior of breast cancer, which can provide reference for indirectly assessment of prognosis and clinical treatment planning.
