摘　　要：目的构建适用于手术，创伤患者预防下肢深静脉血栓形成的风险评估条目，为各级护理人员快速识别风险、及时采取措施、标准规范预防提供可操作的工具。方法采用Delphi法对18名临床及护理专家进行函询调查，最终确定手术/创伤患者预防下肢深静脉血栓评估表的结构、评估条目分级标准和预防措施。结果两轮专家咨询表回收率分别为81．82％和100％，专家积极系数100％；专家权威系数0．877。通过专家鉴定，确定了临床相关风险因素、患者相关风险因素的内容及分值，以及风险分级推荐的预防方法。结论根据专家咨询的积极系数、权威系数等证明，手术/创伤患者下肢深静脉血栓形成风险评估条目代表性及可信度较高，可用于指导临床护理人员对下肢深静脉血栓的预防。Objective To develop a risk assessment form for deep vein Ttrombosis （DVT） of lower extremity for the patients suffering from trauma or undeergoing operation. Methods 18 clinical and nursing experts are choosen for the study of expert consultation through the Delphi method. Results The valid questionnaires are censured and the structure, categories and prevention measures of the assessment form are made. Conclusion According to the active coefficient and certificate of authority coefficient of experts, risk assessment scale for deep vein thrombosis of lower extremity in operation/trauma patients can be used to guide clinical nursing staff for prevention of deep vein thrombosis of.
