摘  要:目的分析河南驻马店市经性传播的艾滋病病毒(HIV)感染者中,HIV流行毒株亚型及其毒株特征。方法通过专业问卷调查,了解驻马店地区经性传播HIV感染者的基本状况。采集感染者的EDTA抗凝静脉血,检测其CD+4T淋巴细胞(简称CD4细胞)计数,提取外周血浆中病毒核糖核酸(RNA),使用反转录巢式荧光定量聚合酶链反应(PCR)方法扩增病毒全长gag和部分pol基因,利用Clustal软件比对序列,利用BioEdit软件手工校对,利用NCBI网站HIV分型工具分析病毒亚型,利用MEGA软件构建NJ树,分析病毒系统进化关系。结果共收集到HIV感染者血液74份,全部经性传播感染HIV。经过测序,共获得HIV全长gag基因56条(75.7%),全长pol基因38条(51.4%)。对61例HIV感染者感染病毒亚型进行了分析,其中B亚型毒株47例(77.0%),CRF01_AE毒株6例(9.8%),CRF07_BC毒株4例(6.6%),C亚型毒株2例(3.3%),新型重组毒株2例(3.3%)。系统进化分析显示,所有的B亚型毒株均为Thai B毒株。38例获得pol区序列的样本中,有4例具有耐药突变。结论驻马店市性传播HIV感染者中流行的HIV毒株构成复杂,Thai B亚型毒株是优势毒株,CRF01_AE、CRF07_BC、C等亚型毒株均已进入该人群,而且出现了新型重组毒株。针对该人群的HIV流行毒株监测至关重要。Objective To analyze subtypes of HIV epidemic in sexual transmitted population in Zhumadian area and to investigate genetic characterizations of those strains.Methods Individuals living in Zhumadian area who were infected by HIV through sexual transmission were recruited.Professional questionnaire was used to collect the general information.Peripherial blood was collected for the test of HIV infection and CD4 Tcell number.Peripherial plasma was used to extract viral RNA.Nested RT-PCR was used to amplify HIV full length gag and pol genes.Clustal software was used to align sequences before manual edited with BioEdit software.Online HIV subtyping software at NCBI website was used to subtyping obtained HIV sequences.Neighbor-Joining tree was constructed with MEGA software.The phylogenetic relationship was further explored basing on the phylogenetic tree.ResultsA total of 74 individuals were enrolled.56 gag genes(75.7%)and 38 pol genes(51.4%)were successfully obtained.Basing on those sequences,the subtypes of HIV strains from 61 individuals were determined,including 47 subtype B strains,6CRF01_AE strains,4CRF07_BC strains,2subtype C strains,2unique recombinant from strains.Phylogenetic analysis showed that all of B subtype strains were Thai B strains. Among 38 individual from whom pol genes were successfully obtained,4strains contained drug-resistant mutations.Conclusion HIV subtypes epidemic in Zhumadian area were complicated.Thai B was the dominant subtype,while CRF01_AE,CRF07_BC and subtype C were all introduced into the population.Furthermore,unique recombinant forms of HIV were identified in the area.Surveillance of HIV in the population in Zhuamadian area is necessary for HIV prevention and control.
