老年牙缺失患者植体周牙龈状况及植体周齿槽骨吸收水平研究

摘　　要：目的探讨老年牙缺失患者植体周牙龈状况及植体周齿槽骨吸收水平。方法回顾性分析2012年2月至2014年2月在青海省人民医院实施牙缺失治疗的176例患者的临床资料,按年龄不同分为老年组(94例)和非老年组(82例),并在术后随访1年。对两组患者的成功率、植体周牙龈状况及植体周齿槽骨吸收水平进行评价。结果两组患者种植体成功率均为100.0%,差异无统计学意义(P>0.05);老年组植体周牙龈状况较非老年组差,差异有统计学意义(P<0.05)。两组患者齿槽骨吸收水平均降低,术后1个月老年组患者的骨吸收水平高于非老年组,之后在术后3、6、12个月,两组患者吸收水平的差异逐渐减小。不同时点间、组间·不同时点间的交互作用比较差异有统计学意义(P<0.05)。结论老年牙缺失患者的植体周牙龈状况及植体周齿槽骨吸收水平均较非老年人差,但其对手术成功率影响不明显。Objective To study the status of gingival and bone resorption surrounding the implant of the elderly patients with tooth loss. Methods The clinical data of 176 patients with tooth loss treated in Qinghai People＇s Hospital from Feb. 2012 to Feb. 2014 were retrospectively analyzed. The 176 eases were divided into elderly groups（94 cases） and non-elderly group（82 cases） ,and followed up for 1 year. The success rate of the implants and crowns, status of gingival and bone resorption surrounding the implant of the two groups were analyzed and compared. Results Implant success rate in both groups was 100.0% , the difference was not statistically significant （ P 〉 O. 05 ） ; the excellent ratio surrounding the implant in the elderly group was low- er than that of the non-elderly group, the good gingival condition, ratio was higher than the non-elderly group, the gingival condition of the elderly group was poor, the difference was statistically significant （ P 〈 0. 05 ）. Alveolar bone absorption level showed a stable trend of decreased first and then increased in the two groups, postoperative 1 month bone absorption level in the elderly group was significantly higher than that of the non- elderly group, at postoperative 3,6 and 12 months, the difference of absorption level in the two groups gradu- ally decreased. There were significant differences in the interaction between different time points, different groups and different time points （ P 〈 0.05 ）. Conclusion The status of gingival and bone resorption sur- rounding the implant of the elderly patients is all worse than those of young people, but it has no obvious effect on the success rate of surgery.
