喷雾联合棉棒擦洗对婴幼儿化疗后口腔黏膜炎影响

摘　　要：目的探讨口腔喷雾联合棉棒擦洗对婴幼儿化疗后口腔黏膜炎的影响。方法化疗后婴幼儿180例,按随机数字表法分为两组。对照组90例,采用传统的含漱方法漱口;实验组90例,采用口腔喷雾联合棉棒擦洗口腔黏膜。比较两组病儿漱口时的配合情况、口腔黏膜炎的发生例数、发生状况、发生程度、发生时间、愈合时间及口腔护理后致病性微生物检测阳性例数等方面的差异。结果实验组在漱口时的配合方面优于对照组,口腔黏膜炎发生例数较对照组少（χ2=5.16~41.05,P〈0.05）,且发生程度轻（Z=2.95,P〈0.05）,主要以单发为主,而对照组以多发为主;实验组口腔溃疡发生时间晚于对照组,愈合时间明显短于对照组（t=2.17、5.01,P〈0.05）;口腔护理后致病性微生物检测阳性例数少于对照组（χ2=8.75,P〈0.05）。结论口腔喷雾联合棉棒擦洗可以有效地预防婴幼儿化疗后口腔黏膜炎的发生,降低口腔黏膜炎的发生程度,缩短愈合时间。Objective To investigate the efficacy of spraying combined with cotton swabbing for stomatitis in infants and young children after chemotherapy. Methods This study enrolled 180 infants and young children after chemotherapy and evenly randomized to two groups as experiment group and control group.Ninety cases in the control group were treated with traditional mouthwash to rinse the mouth;those in the experiment group were offered oral spraying plus cotton swabbing to scrub oral mucosa.The differences between the two groups were compared in the following items：cooperation of gargling,incidence of stomatitis,its status and its degree,time of occurrence,healing time,and cases of positive invasive organism after mouth care. Results The compliance of gargling in sick children was better and the incidence of stomatitis was lower（χ2=5.16-41.05,P〈0.05）,with milder degree（Z=2.95,P〈0.05）in the experiment group than that in the control.The ulcers were mainly single in the experiment group versus multiple in the control.The mouth ulcer in the experiment group occurred later than that in the control and heal time was shorter（t=2.17,5.01;P〈0.05）.The positive cases of pathogenic microorganism was less in the experiment group versus the control（χ2=8.75,P〈0.05）. Conclusion Combination of oral spraying with cotton swabbing can effectively preventing the occurrence of stomatitis in infants and young children after chemotherapy,decrease the degree of the lesion and shorten its healing time.
