儿童神经母细胞瘤综合治疗疗效分析

摘　　要：目的对42例神经母细胞瘤（NB）患儿治疗的长期疗效进行分析,以寻求提高NB患儿,尤其是高危NB患儿长期无病生存率的方法。方法回顾性分析2000年1月-2014年6月我院儿科血液/肿瘤病房收治的42例NB患儿的临床资料,中位年龄32（3-93）个月,其中Ⅰ、Ⅱ、ⅣS期10例,Ⅲ、Ⅳ期32例,采用KapIan—Meier方法进行长期生存情况分析,并对复发NB患儿的情况进行分析。结果Ⅰ、Ⅱ、ⅣS期NB患儿的3年无病生存率可达80.2%,Ⅲ、Ⅳ期NB患儿的3年无病生存率仅为28.1%。复发5例,其中Ⅲ期2例,Ⅳ期3例。二次肿瘤发生1例。结论高危NB患儿长期无病生存率的提高需要采取综合治疗方案,包括大剂量化疗、局部或靶向放疗、手术切除、造血干细胞移植、免疫疗法等的综合运用。Objective To analyze the treatment response in 42 childhood neuroblastoma patients,and explore how to improve the event free survival rate in neuroblastoma,especially in high-risk ones.Methods 42 children diagnosed with neuroblastoma from January 2001 through June 2014 were analyzed retrospectively.The median age was 32 months（from 3 to 93 months）.Accroding to the The International Neuroblastoma Staging System,10 cases were at stage Ⅰ、 Ⅱ and Ⅳs,32 cases were at stage Ⅲ and Ⅳ.Kaplan—Meier method was used to evaluate EFS.And relapsed neuroblastoma cases were analyzed.Results The 3-year EFS rate in stage Ⅰ、Ⅱ and IVS was 80.2%,in stage Ⅲ and Ⅳ it was only 28.1%.Relapse occurred in 5 cases,in which 2 cases were stage Ⅲ and 3 cases were stageⅣ.There was 1 patient who had recurrent neoplasms.Conclusions Multidisciplinary therapy protocol including high-dose chemotherapy,local/targeted radiotherapy,surgery,hematopoietic stem cell transplantation,and immunotherapy,can improve the EFS rate in high-risk neuroblastoma.
