中西医结合治疗晚期非小细胞肺癌临床优化方案研究

摘　　要：目的:观察中西医结合治疗晚期非小细胞肺癌(NSCLC)的临床疗效,以建立晚期非小细胞肺癌中西医结合临床优化治疗方案。方法:134例晚期NSCLC患者分为4组,正虚对照组(33例)、邪实对照组(33例)予顺铂(DDP)+多西紫杉醇(Doc)化疗;正虚观察组(34例)、邪实观察组(34例)在化疗基础上,正虚为主型予参芪扶正注射液和芪贞固本方;邪实为主型予苦参碱注射液和芩莲正积方。两周期后,比较两组患者治疗效果,观察1年,比较1年生存率。结果:与对照组相比,各观察组可明显提高肿瘤控制率、增加卡氏评分和体质量、降低症状积分和肿瘤标志物(CEA、CA19-9)、提高1年生存率、减轻消化道反应(P<0.05);扶正观察组与其对照组相比可明显增强患者免疫功能,减轻骨髓抑制(P<0.05)。结论:本研究所采用中西医结合治疗方案具有良好的治疗效果,能减毒增效,与单纯化疗相比有显著优势,值得临床广泛推广。Objective： To observe the clinical efficacy of treatment with integrated therapy of traditional Chinese and western medicine and establish the optimal treatment of advanced NSCLC by integrative medicine. Method： 134 patients with advanced NSCLC were divided into four groups. The control groups were given DP program. On the basis of the above,in the combined therapy groups,the therapy added Chinese patent drug and decoction of supporting healthy or removing evil for corresponding etiology and pathogenesis. The effect was evaluated after 2 cycles of chemotherapy. Results：Compared to control groups,the observation groups could enhance the tumor control rate,increase the KPS and weight,reduce symptom score and the level of CEA、CA- 199,raise the 1 years survival rates and ease the adverse reaction of digestive system significantly（ P〈0. 05）. And compared to its control group,the observation group of deficiency type could enhance the immune function and lighten hematological adverse reaction significantly（ P〈0. 05）. Conclusion： The scheme in our research had a good therapeutic result which could reduce the toxicity and enhance the efficacy. It had a significant advantage compared to chemotherapy only and it deserves the clinical expansion.
