鸦胆子油胶囊联合顺铂胸腔内灌注治疗肺癌所致恶性胸腔积液的临床观察

摘　　要：目的:探讨中成药鸦胆子油胶囊联合顺铂胸腔内灌注治疗肺癌所致恶性胸腔积液的临床观察。方法:66例肺癌所致恶性胸腔积液患者,采用随机数字表法将其分为观察组与对照组各33例。留置导管排尽胸水后,对照组单纯给予顺铂胸腔内灌注治疗,观察组在对照组治疗基础上,外加给予鸦胆子油胶囊治疗。治疗后对两组临床疗效、生活质量改善情况及不良反应情况进行比较。结果:观察组患者治疗总有效率为78.8%,较对照组临床总有效率为54.5%明显提高,差异有统计学意义(P<0.05);治疗前,两组患者的生活质量评分比较差异无统计学意义(P>0.05);治疗后,两组患者的生活质量评分均明显提高,差异具有统计学意义(P<0.05),与对照组比较,观察组患者治疗后生活质量评分均明显提高,差异具有统计学意义(P<0.05)。观察组患者生活质量改善率为75.8%,较对照组生活改善率为51.5%明显提高,差异有统计学意义(P<0.05);与对照组比较,观察组不良反应发生率明显降低,差异均有统计学意义(P<0.05)。结论:鸦胆子油胶囊联合顺铂胸腔内灌注治疗肺癌所致恶性胸腔积液具有显著临床疗效,且不良反应少,值得临床推广应用。Objective： To evaluate the clinical observation of Brucea Javanica Capsule and cisplatinum treating lung cancer malignant pleural effusion. Methods： 66 cases of patients suffering from lung cancer malignant pleural effusion form January 2013 to February 2015 in our hospital were selected and divided into two groups randomly. 33 patients in control group were treated with cisplatinum by intrathoracic injection while the other 33 patients of the observation group were treated with Brucea Javanica Capsule and cisplatinum by intrathoracic injection. After treatment,we compared the clinical efficacy,quality of life and side- effects between the two groups. Results： The effective rate was 78. 8% in observation group and higher than 54. 5% in the control group and the difference was significant（ P 〈0. 05）. Before treatment,the patients＇ quality of life between the two groups was similar and had no statistical significance（ P 〉0. 05）. After treatment,two groups＇ patients quality of life scores were significantly improved（ P 〈0. 05） and the control and observation groups＇ difference was statistically significant（ P 〈0. 05）. The improvement rate of life quality in observation group was75. 8% and higher than the control group＇s（ 51. 5%） and the difference was significant（ P〈 0. 05）. The side effects in observation group were less than the control group＇s and the difference was significant（ P〈 0. 05）. Conclusion： Brucea Javanica Capsule and cisplatinum in the treatment of lung cancer malignant pleural effusion have a significant clinical efficacy and the side effects are less. It has the value of clinical application.
