青光眼白内障联合手术的疗效观察

摘　　要：目的：分析总结青光眼白内障联合手术治疗急性闭角型青光眼合并白内障患者方法及疗效。方法对30例（30眼）急性闭角型青光眼合并白内障患者行晶状体超声乳化吸出、人工晶状体植入联合小梁切除术,观察术后反应、视力、眼压情况。结果术后平均眼压1个月、6个月、1年分别为（13.1±;1.3）、（12.6±;0.6）、（11.4±;0.6）mm Hg（1 mm Hg=0.133 kPa）,均较术前明显降低（P＜0.01）,且术后1年的随访中,均未再应用任何降眼压药物。30例（30眼）术后最佳矫正视力0.6～0.8者17例,0.4～0.5者10例,0.1～0.3者3例。结论青光眼白内障联合手术疗效确切,术前积极控制眼压,术后及时处理并发症,可获得良好疗效。Objective To analyze and summarize method and curative effect of combined surgery for glaucoma and cataract in the treatment of acute angle-closure glaucoma complicated with cataract patients. Methods A total of 30 patients （30 eyes） with angle-closure glaucoma and complicated cataract received phacoemulsification, intraocular lens implantation and trabeculectomy in combined surgery. Their postoperative reactions, vision and intraocular pressure were observed.Results The average postoperative intraocular pressure in 1 month, 6 months and 1 year were respectively （13.1±;1.3）, （12.6±;0.6）, and （11.4±;0.6） mm Hg （1 mm Hg=0.133 kPa）. They were all obviously lower than that before operation （P〈0.01）. Follow-up for 1 year after operation showed no administration of any intraocular pressure-lowering agents. Among the 30 cases （30 eyes）, 17 cases of them had the best corrected visual acuity as 0.6～0.8, 10 cases had it as 0.4～0.5, and 3 cases as 0.1～0.3.Conclusion Combined surgery for glaucoma and cataract provides precise effect. Active preoperative control of intraocular pressure and timely postoperative management of complications can provide good curative effect.
