骨吸收和功能牙列对口腔健康生活质量的影响?剻?

摘　　要：目的了解牙槽骨吸收状况与不同功能牙列类型的相关性,评估余留牙牙周健康状况对不同功能牙列病人口腔健康相关生活质量(OHRQoL)的影响。方法随机选取临床病人130例,对其进行口腔临床检查和口腔健康影响程度量表(OHIP-14)中文版问卷调查,同时对病人牙槽骨吸收率(BL)进行测量。按照Nijmegen功能等级分类系统对病人牙列进行功能分类,分析牙槽骨水平对不同功能牙列病人OHRQoL的影响。结果多重回归分析显示,年龄对最低BL的影响有统计学意义(B=-0.269,P<0.001)。各功能等级中,颌骨水平与最低BL显著相关(B=5.511,P<0.05)。OHIP-14总分与年龄(B=0.236,P<0.05)、最低BL(B=-0.328,P<0.05)、前牙完整(B=-4.639,P<0.05)以及前磨牙足够的咬合对数(B=-6.227,P<0.05)具有显著相关性。结论上下颌均有10颗以上牙齿有助于减少牙槽骨的吸收。病人健康的牙周状况和有完整的前牙列、至少3个前磨牙咬合对数对维持病人良好的OHRQoL至关重要。Objective To investigate the correlation between alveolar resorption and different-type dentition and assess the impact of periodontal health of remaining teeth on oral health-related quality of life（OHRQoL）in patients with different functional dentitions. Methods A total of 130 patients were clinically examined for their oral conditions,and a questionaire survey on oral health OHIP-14（Chinese version）was used to survey,the rate of alveolar bone loss（ABL）was detected at the same time.The dentitions were classified based on the Nijmegen functional classification system,the impact of different alveolar bone levels on OHRQoL of the patients was analyzed. Results Multiple regression analysis results showed the impact of age on the lowest ABL had a statistical significance（B=-0.269,P〈0.05）.In each functional class,the jaw level was significantly correlated with the lowest ABL（B=5.511,P〈0.05）,the total score of OHIP-14 was significantly correlated with age（B=0.236,P〈0.05）,lowest ABL（B=-0.328,P〈0.05）,anterior teeth integrity（B=-4.639,P〈0.05）and premolar sufficient occlusion pairs（B=-6.227,P〈0.05）. Conclusion Having at least 10 teeth in each jaw can reduce the absorption of the alveolar bone.A healthy periodontal condition and complete anterior teeth at least with three bicuspid occluding pairs in premolar region is extremely important in keeping apatient＇s satisfactory oral health-related quality of life.
