生酮饮食治疗儿童难治性癫痫的疗效与依从性分析

摘　　要：目的分析生酮饮食(KD)治疗儿童难治性癫痫的疗效与依从性。方法采用经典的KD方案治疗41例难治性癫痫病儿,记录病儿接受KD治疗前及治疗3个月期间的丙戊酸血浓度、服药剂量及体质量的变化,观察癫痫发作控制情况。结果经3个月随访,41例病儿中有26例完成观察,其中4例完全控制发作,16例发作减少50%以上,6例病儿发作无明显变化,总有效率为76.92%。丙戊酸血浓度在治疗前后未发生显著改变(F=0.66,P>0.05)。KD治疗的保留率为83.87%。结论生酮饮食治疗儿童难治性癫痫安全、有效,副作用少,提高病儿和家长的依从性是改善治疗效果的关键。Objective To analyze the efficacy and compliance of ketogenic diet in treating refractory childhood epilepsy.Methods Employing classical ketogenic diet program,41 children with refractory epilepsy were treated.The blood concentration of valproic acid,changes of drug dose,body weight,and seizure control,before and three months after treatment,were observed.Results Twenty-six of the sick kids completed a three-month follow-up,of whom,the attack was completely controlled in four kids,the attack reduced over 50% was noted in 16,and six with no obvious changes of the attack,the total effective rate being76.92%.The blood concentrations of valproic acid were not significantly changed before and after therapy（F=0.66,P〈0.05）.Retention rate of ketogenic diet therapy was 83.87%. Conclusion Ketogenic diet is safe,effective,and little side effects for children with refractory epilepsy.The key of improving treatment effectiveness is to boost the compliance of the sick kids and their parents.
