冲顶式上颌窦内提升术不植骨同期植入短种植体的观察

摘　　要：目的：评估冲顶式上颌窦内提升术不植骨同期植入短种植体的临床疗效。方法：25例患者接受冲顶式上颌窦内提升术，同期植入奥齿态短种植体（共31枚，8.5mm长21枚,7mm长10枚）。术后3~6个月行种植义齿修复，术后2年进行临床及X线检查随访。结果：种植体术后2年成功率为99.7％，种植体稳定且能承担咬合功能。X线片显示所有种植体底部及周围均有硬组织包绕，且牙槽嵴顶水平稳定，垂直骨吸收量〈1mm。结论：冲顶式上颌窦内提升术不植骨同期植入短种植体能取得较好的临床效果，拓宽了上颌后牙区骨量不足的种植修复适应证，远期效果有待进一步观察。Objective：The aim of the present study was to evaluate the practicability of osteotome sinus floor elevation without grafting and with simultaneous short implants placement. Methods：5 patients received 31 implants protruding into the sinus.Most implants（21/31）were 8.5 mill in length,the other 10 were 7 mill.A healing period of 3 or 6 months was scheduled.Radiographic evaluation of marginal bone around the implant was performed before and after surgery.The follow—up period was 2 years.Results：The implants success rate was 99.7%.X-ray examination showed all implants were surrounded with hard tissues and the marginal bone around the implants were stable,the vertical quantity of the absorbed bone Was less than 1 mm. Conclusion：Elevation of the sinus membrane alone without addition of bone graft material is less invasive.Using short implants also broaden the inclusion criteria which the posterior maxilla bone is not sufficient.We still need to follow—up longer to get more precise results.
