摘　　要：目的探讨临床药师对T细胞淋巴瘤相关嗜血细胞综合征患者实施药学监护的过程、方法及作用。方法对1例行"培门冬酶联合COP方案"化疗的T细胞淋巴瘤相关嗜血细胞综合征患者提供药学监护,对化疗前进行方案实施评价分析,提出药学监护点,同步开展患者用药教育。结果临床药师通过对药效的治疗前、治疗中的药学监护,可为患者提供个体化用药,保证化疗疗效,减少药源性损害。结论临床药师应利用与医护人员互补的药学知识为切入点,积极开展个体化的药学监护。Objective To investigate the processes,methods and effects of clinical pharmacists-conducted pharmaceutical care on the patients with T-cell lymphoma associated hemophagocytic syndrome （T-LAHS）. Methods Aiming at the pegaspargase and COP chemotherapy,the pharmaceutical care was provided,the chemotherapy regimen evaluation was performed,and the medication education on the patients was carried out. Results The pharmaceutical care provided by clinical pharmacists made an individualized pharmaceutical care for the patients,which can improve the therapeutic effects and minimize the adverse effects. Conclusion Clinical pharmacists should take advantage of pharmaceutical knowledge as the point cut,offering individualized pharmaceutical care for patients.
