凉山地区女性宫颈感染人乳头瘤病毒基因型分析

摘　　要：目的分析凉山地区妇女宫颈上皮细胞人乳头瘤病毒的感染情况及其主要型别。方法应用反向斑点杂交-基因芯片技术,对2014年1至8月就诊的2643例女性患者宫颈脱落细胞进行23种基因分型检测。结果 2643份标本检出阳性669例,HPV感染率25.31%。其中单一感染检出476例,感染率18.01%;两种及以上亚型感染检出193例,感染率7.30%;族别统计958例,其中汉族阳性率24.16%(172/712),彝族阳性率29.15%(65/223),民族分析差异无统计学意义(=2.24 P>0.05)。检出23种HPV基因亚型,其中低危型阳性167例,以81型阳性最多;高危型阳性763例,阳性例数排前五位的为:52、16、58、53及18型。HPV阳性检出率以≤20岁(38.78%)及>51岁(33.21%)年龄段较高,感染检出率比较差异有统计学意义(=22.99,P<0.01)。结论凉山地区女性宫颈HPV感染具有独特的地域分布和年龄分布特点,族别之间没有差异。Objective To study infection situation and distribution of human papilloma virus（ HPV） genotype from women with uterine cervical epithelial cells in Liangshan Area. Methods 2643 patients＆#39;s uterine cervical cast-off cells from different age in January-August 2014 were detected and typed to 23 genotypes by dot blot hybridization reverse-gene chip. Results The posi-tive rate of HPV from 2643 patients was 25. 31%, about 699 patients;the single positive infection was 18. 01%, about 476 pa-tients;while the infection of two or more kinds of HPV subtypes was 7. 30%,about 193 patients. Statistics 958 cases depended on ethnicity. The positive rate of the Han people was 24. 16%（172/712）,Yi people was 29. 15%（65/223）,There was no significant difference in the national analysis. From 23 genotypes of HPV,the positive patients of low-risk types were 167,among them the 81 genotype was most;the positive rate of high-risk types were 763,the top five positive cases were 52 genotype、16 genotype、58 geno-type、53 genotype and 18 genotype. Moreover,the patients with positive HPV in the age group ≤20 years（38. 78%） and ≥ 51 years（33. 21%）were higher. Infection detection comparative difference was statistically significant（ =22. 99 P〈0. 01）. Con-clusion Female uterine cervical HPV infection had a unique geographical and age distribution characteristics in Liangshan Area, There was no significant difference between ethnicity.
