角膜塑形术治疗青少年近视的长期临床观察

摘　　要：目的探讨角膜塑形术治疗青少年近视的长期疗效及安全性。方法连续配戴角膜塑形镜2年以上的51例（102只眼）青少年近视患者,平均年龄（11.63±1.91）岁,戴镜前平均等效球镜（-3.71±1.32）D,采用夜间戴镜8～10 h,观察戴镜前和戴镜后1周、1、3、6个月、1、2年的裸眼视力、角膜曲率,以及戴镜后1、2年的屈光度、矫正视力的变化,同时观察角膜并发症的情况。结果戴镜后2年,裸眼视力从戴镜前0.21±0.09提高到0.99±0.10（P〈0.01）;角膜曲率明显变平坦,平均K值从（43.27±1.16）D下降为（40.69±1.30）D（P〈0.01）;戴镜前与戴镜后1、2年,矫正视力维持在1.0（P〉0.05）;等效球镜分别为（-3.71±1.32）D、（-3.74±1.33）D、（-3.87±1.31）D,平均每年增长0.08D,差异无统计学意义（P〉0.05）。未见严重的角膜并发症。结论科学规范的长期配戴角膜塑形镜安全可靠,可有效地提高患者裸眼视力,在一定程度上可控制近视发展。Objective To observe the long-term effects and safety of orthokeratology（ Ortho-K,OK） in young myopia patients. Methods Author enrolled 51 adolescent patients（ 102 eyes） with myopia who wore OK lens over 2 years period. The average age was 11. 63 ± 1. 91 and the average spherical equivalent refraction was- 3. 71 ± 1. 32 diopters（ D）. All patients were fitted with overnight OK for 8 ～ 10 hours at night. The visual acuity,refraction,corneal curvature and complications were recorded at pre-wearing,1 week,1 month,3 months,6 months,1 year,and 2 years post-wearing.Results Uncorrected visual acuity（ UCVA） significantly improved from 0. 21 ± 0. 09 before wearing to 0. 99 ± 0. 10 at 2years（ P〈0. 01）. Average K value of corneal curvature reduced from 43. 27 ± 1. 16 D to 40. 69 ± 1. 30D（ P〈0. 01）.At pre-wearing,1 year and 2 years,corrected visual acuity（ CVA） was stabilized at 1. 0. Spherical equivalent refraction（ SER） was- 3. 71 ± 1. 32 D,- 3. 74 ± 1. 33 D,and- 3. 87 ± 1. 31 D respectively. There were no significant changes in CVA and SER（ both P〉0. 05）. Average SER increased 0. 08 D every year. There was no severe complication in the ocular surface. Conclusion Standardized long-term wearing OK is safe and is effective to improve UCVA and control the progression of myopia.
