摘　　要：目的探究DCE-MRI对乳腺癌患者预后评估的价值。方法选取60例MR扫描前未进行抗肿瘤治疗的乳腺癌患者,术前给予DCE-MRI检查,记录最大强化率(SImax)、时间-强度曲线(TIC)、最大线性斜率(slope)和达峰时间(Tpeak)相关参数;病理组织免疫组化学染色记录预后因子Ki-67、c-erb B-2、ER、PR、P53的表达情况。行卡方检验探究预后因子与乳腺癌预后的关系和行直线相关分析探究DCE-MRI相关系数和预后因子的关系。结果 Ki-67、c-erb B-2和P53表达与乳腺癌临床分期有关(P<0.05),而ER、PR的表达与乳腺癌临床分期无关;c-erb B-2和P53阳性表达与乳腺癌淋巴转移有关;而Ki-67、c-erb B-2、ER、PR、P53表达均与肿瘤大小无关。DCE-MRI的Tpeak与ER和PR的阳性表达呈正相关(γ=0.541,P<0.05;γ=0.552,P<0.05),Ki-67、c-erb B-2和P53的阳性表达与Tpeak呈负相关(γ=-0.532,P<0.05;γ=-0.534,P<0.05;γ=-0.526,P<0.05);SLmax、Slope、TIC与各预后因子的相关性无统计意义。结论预后因子Ki-67、c-erb B-2、ER、PR、P53与乳腺癌患者预后相关,而DCE-MRI的Tpeak与各种预后因子紧密相关,可有效评估乳腺癌患者的预后。Objective To explore the evaluation value of DCE-MRI on the prognosis of breast cancer.Methods 60 cases of breast cancer who underwent DCE-MRI before operation were selected.The washout ratio（SImax）,type of signal intensity curve（TIC）,intial slope（slope） and time to peak（Tpeak） were recorded.Ki-67,c-erb B-2,ER,PR and P53 were analyzed by clinicopathological immunohistochemical staining.Results The biomarkers（ER,c-erb B-2,Ki-67,PR and P53） were correlated with the prognosis of breast cancer.Tpeakwas negatively correlated with the expression of Ki-67、c-erb B-2 and P53（γ =-0.532,P〈0.05; γ =-0.534,P〈0.05; γ =-0.526,P〈0.05）.It also was positively correlated with the expression of ER and PR（γ =0.541,P〈0.05; γ = 0.552,P〈0.05）.Conclusion Some dynamic parameters of DCE MRI are useful in evaluating the prognosis and drawing up the treatment plan of the breast carcinoma.
