丙氨酰-谷氨酰胺对胰腺癌术后肠道通透性与免疫功能的作用分析

摘　　要：目的分析丙氨酰-谷氨酰胺(Aln-Gln)对胰腺癌术后肠道通透性与免疫功能的改善作用。方法选取行胰腺癌手术的患者38例,随机分为观察组和对照组,各19例。观察组术后给予Aln-Gln治疗,对照组术后给予同等容量的0.9%氯化钠注射液作为安慰剂。比较两组患者术前1 d、术后第3、5 d血浆Gln浓度,术前1 d、术后第5 d的血浆二胺氧化酶(DAO)浓度和尿乳果糖/甘露醇(L/M)比值,术前1 d、术后第1、5 d的HLA-DR和TLC。结果观察组术后第3、5 d血浆Gln浓度明显高于术前(P<0.05),同时也明显高于对照组术后(P<0.05);观察组术后第5 d血浆DAO浓度明显低于术前(P<0.05),同时也明显低于对照组术后(P<0.05);观察组术后第5 d尿L/M比值与术前相比差异无统计学意义(P>0.05),同时明显低于对照组术后(P<0.05);两组患者术后第5 d的HLA-DR和TLC明显高于术后第1 d(P<0.05),其中观察组明显高于对照组(P<0.05)。结论 Aln-Gln应用于胰腺癌术后可有效提高并维持血浆Gln浓度,维护肠屏障功能,改善其免疫功能,有利于患者术后康复。Objective To analyze propylene ammonia acyl-glutamine（Aln-Gln） on postoperative intestinal permeability and the improvement of immune function of pancreatic cancer.Methods 38 cases of patients with pancreatic cancer treated with surgery,were randomly divided into the observation group and the control group,19 cases in each.The observation group received postoperatively Aln-Gln treatment,the control group received equal volume of 0.9% sodium chloride injection as a placebo.Plasma concentrations of Gln preoperative 1 d and postoperative 3 d,5 d of the 2 groups were compared,plasma diamine oxidase（DAO） concentration and urinary lactulose/mannitol（L/M） ratio preoperative 1 d and postoperative 5 d of the 2 groups were compared,HLA-DR and TLC preoperative 1 d and postoperative 1 d,5 d of the 2 groups were compared.Results In the observation group,postoperative 3 d,5 d plasma Gln concentration were significantly higher than preoperative（P〈0.05）,and also were significantly higher than those of the control group（P〈0.05）; Postoperative 5 d plasma concentrations of DAO in the observation group was obviously lower than preoperative（P〈0.05）,and also significantly lower than those of the control group（P〈0.05）;Postoperative 5 d in the observation group urine L / M ratio had no statistically significant difference compared with preoperative（P〈0.05）,and also was significantly lower than that of the control group（P〈0.05）; 2 groups of patients with postoperative 5d HLA-DR and TLC were significantly higher than those of postoperative 1 d（P〈0.05）,the observation group were significantly higher than the control group（P〈0.05）.Conclusion Aln-Gln in pancreatic cancer surgery can effectively improve and maintain the plasma concentration of Gln,maintenance of intestinal barrier function,can improve the immune function,is beneficial to patients with postoperative rehabilitation.
