低频重复经颅磁刺激联合文拉法辛治疗广泛性焦虑障碍的早期疗效

摘　　要：目的探讨低频重复经颅磁刺激(rTMS)联合文拉法辛治疗广泛性焦虑障碍的早期临床疗效。方法将40例首发广泛性焦虑障碍患者随机分成两组,研究组采用r TMS联合文拉法辛治疗,对照组单用文拉法辛治疗。分别于治疗前及治疗后第1、2周末应用汉密尔顿焦虑量表(HAMA)及焦虑自评量表(SAS)评定临床疗效,采用治疗中需处理的不良反应症状量表(TESS)评定不良反应。结果研究组和对照组HAMA总分及各因子分、SAS总分在治疗后第1、2周末较治疗前均下降(P<0.05)。研究组HAMA总分及各因子分、SAS总分在治疗后第2周末低于对照组(P<0.05)。研究组显效率在治疗后第2周末高于对照组(P<0.05)。两组间不良反应发生率比较无显著性差异(P>0.05)。结论低频重复经颅磁刺激联合文拉法辛治疗广泛性焦虑障碍疗效好,起效快,优于单一用药。Objective To explore the early effectiveness of low frequency repetitive transcranial magnetic stimulation（ rTMS） combined with venlafaxine in treatment of patients with generalized anxiety disorder（GAD）.Methods A total of 40 patients with GAD were randomly divided into study group treated with rTMS combined with venlafaxine and control group treated with venlafaxine monotherapy.They were assessed with Hamilton Anxiety Scale （HAMA）, Self-Rating Anxiety Scale （SAS） and Treatment Emergent Symptom Scale （TESS） at baseline and at the 1st, 2nd weekend of treatment.Results At the 1st and 2nd weekend of treatment, the total scores of HAMD and SAS as well as all factor scores of HAMA in two groups were all significantly lower than those before treatment（P〈0.05）.At the 2nd weekend of treatment, the total scores of HAMD and SAS as well as all factor scores of HAMA in study group were all significantly lower than those in control group（P〈0.05）.At the 2nd weekend of treatment, the effective rate in study group was significantly higher than that in control group（P〈0.05）.There was no difference for incidence of side effects between two groups（P 〉0.05）.Conclusion It’s effective and rapid for rTMS combined with venlafaxine in the treatment of patients with GAD, which is better than venlafaxine monotherapy.
