老年女性抑郁症患者的临床特征分析

摘　　要：目的:探讨老年女性抑郁症患者的临床特征。方法:对符合入组标准的143例老年女性抑郁患者,将其分为老年期首发抑郁组(老年首发组)和老年期复发抑郁组(老年复发组),使用自制调查量表对两组患者的发病诱因、临床表现、月经史、躯体合并症及疗效进行记录及对照分析。结果:老年女性抑郁症患者起病前有发病诱因者高达75.5%,老年首发组存在发病诱因的比例明显高于老年复发组(P<0.05);临床表现中躯体化症状发生率高达79.7%,老年首发组患者的躯体化症状、激越和记忆力减退均显著高于复发组,差异均有统计学意义(P<0.05);有高达89.5%的抑郁患者合并躯体疾病;老年复发组绝经年龄明显早于老年首发组,差异有统计学意义(P<0.05);老年女性抑郁症治愈率为30.8%,老年首发组治愈率明显低于老年复发组,差异有统计学意义(P<0.05)。结论:老年女性抑郁患者起病前有明显的诱发因素,并存在躯体化症状多、躯体疾病共病率高、治愈率低等特点,尤其以老年期首发患者更为突出。Objective： To explore the clinical features of major depression in elderly women. Methods： The standard set of 143 cases of major depression in elderly women divided into the late onset elderly depression group （the late onset group） and the recur elderly depression group （the recur group）, using self-made survey scale the predisposing factors, clinical manifestation, menstrual history, body complication and curative effect of two groups and contrast analysis. Resuits： The rate of the predisposing factors before the onset of depression in elderly women is as high as 75.5% the proportion of the predisposing factors in the late onset group significantly higher than the recur group（P〈0. 05） ;The rate of somatization symptoms in clinical manifestations is as high as 79.7%, the late onset group of patients with somatic symptoms, agitation and memory impairment were significantly higher than that of recur group, differences were statistically significant（P〈0.05）, with as much as 89.5 % of patients with combined body disease; The age of menopause in the recur group was obviously earlier than the late onset group, the difference was statistically significant（P〈0. 05）; The cure rate of major depression in elderly women is 30. 8% ,which in the late onset group was lower than that in the recur group, the difference was statistically significant（P〈0. 05）. Conclusion： Elderly women before the onset of depression patients have obvious predisposing factors,more somatization symptoms, higher comorbidity rate for the body disease, lower cure rate, especially in the late onset elderly depression patients with a more prominent.
