摘 要:目的探讨携带9号染色体臂间倒位[inv(9)]对不孕症夫妇IVF/ICSI临床结局的影响。方法回顾性分析19对其中一方携带inv(9)的不孕症夫妇作为研究组,158对与这些患者同一天取卵且夫妇双方染色体正常的不孕症夫妇作为对照组,观察两组临床结局,同时比较男方与女方携带inv(9)时治疗结局的差异。结果研究组与对照组的女方平均年龄、男方平均年龄、基础卵泡刺激素(FSH)、基础黄体生成素(LH)、促性腺激素(Gn)总量、Gn天数、HCG日雌二醇(E2)、HCG日孕酮(P)水平、获卵数、受精率、卵裂率、优质胚胎数及内膜厚度均无统计学差异(P>0.05);研究组与对照组比较,胚胎种植率(34.6%vs.51.1%)及临床妊娠率(53.8%vs.66.1%)略低,但无统计学差异(P>0.05),活产率也无统计学差异(53.8%vs50.8%,P>0.05);男方携带inv(9)时精子活率下降[(16.8±15.7)%vs.(31.7±10.0)%,P=0.03)、精子畸形率增加[(87.8±4.8)%vs.(82.7±2.8)%,P=0.02),同时男方携带inv(9)的不孕症夫妇受精方式为ICSI的比例显著高于女方携带inv(9)者(50.0%vs.0%,P=0.01),但两者临床结局无统计学差异(P>0.05)。结论携带inv(9)不影响IVF/ICSI治疗新鲜胚胎移植的妊娠结局,但是携带inv(9)可以影响精子的质量及功能,ICSI比例显著升高。Objective: To explore the effect of chromosome 9 pericentric inversion Einv (9)1 on IVF/ICSI outcomes. Methods: A retrospective analysis was performed in 19 couples with inv (9)as the study group, and 158 normal karyotype couples who were retrieved oocytes on the same day as the control group. The clinical outcomes were compared between the two groups, and also between the couples with female or male inv(9) carrier. Results basal levels : There were no significant differences between two of FSH and LH, gonadotropin(Gn) dosage, Gn da number of retrieved ooeytes, fertilized oocytes endometrial thickness, implantation rate(34.6 % and live birth rate(53.8% vs. 50.8%) (all P〉 significantly decreased [(16.8±15.7)% vs. (31. significantly increased [(87.8±4.8)% vs. (82.7 rate, cleave vs. 51.1%) 0.05). The proportion of ICSI technique used in infertility couples with male inv(9)carrier was significantly higher than that in couples with female inv(9) carrier (50.0% vs. 0%, P = 0.01), but there was no significant difference in clinical outcome between these two groups(P〉0.05). Conclusions: Chromosome 9 pericentric inversion does not impact IVF/ICSI outcomes. Inv(9) can reduce quality and function of sperm. The proportion of ICSI technique used in couples with male inv(9) carrier was significant increased compared to that in couples with female inv(9)carrier.
