摘  要:目的探讨不同加力方式对尖牙远中移动速度及其龈沟液中基质金属蛋白酶-1(matrix metalloproteinase-1,MMP-1)及金属基质蛋白酶抑制剂-1(tissue inhibitor of metalloproteinases-1,TIMP-1)表达的影响。方法选取根据需要拔除了4个第一双尖的24名女性患者随机分为2组,A组:橡皮链组;B组:镍钛拉簧组。每组均施加150g的初始拉力远中移动上下颌尖牙。分别在加力前,加力后1周、2周、4周、6周分别收集右侧上下颌尖牙处的龈沟液、采集上下颌模型。应用ELISA方法测定所取龈沟液中的MMP-1和TIMP-1的浓度,用游标卡尺分别测量上下颌尖牙的移动距离。对所测得的数据进行统计学分析。结果 1AB两组无论上颌还是下颌,均在加力后第1周移动距离最大,差别具有统计学意义。在第2周开始逐渐减慢,到第5、6周时上下颌尖牙几乎停止移动。6周总的移动距离,A组小于B组,差异具有统计学意义(P<0.05)。2A组和B组无论上下颌MMP-1及TIMP-1在加力后浓度变化在加力后1周时,AB两组的上下颌尖牙龈沟液中MMP-1及TIMP-1浓度均开始上升,且达到最高峰,随后开始下降.但B组与A组相比,浓度均变化更加均匀且稳定(P<0.05)。结论 1在相同初始力量的情况下,镍钛拉簧较橡皮链远中移动尖牙移动幅度较大,且产生的生物学效应较持久。2MMP-1及TIMP-1早期即参加了正畸牙周组织改建,且其相互平衡对牙周组织改建以及牙齿的快速移动中起到重要作用。Objective To study the effects of different loading approaches on canine distal movement and expression of MMP-1 and TIMP-1. Methods Twenty four female patients were chosen from the Orthodontic Department of Xiangya 2nd Houspital, CSU as subjects.And divided them into two groups at random,group A: canines were pulled distally by elastic chain with 150 g original force, group B: canines were pulled distally by Ni-Ti coil spring with 150 g original force. Original force were exerted to their maxillary and mandibular canines. The GCF(gingival crevicular fluid,GCF)and gypsum model were collected before loading, 1st week,2nd week 4th week,6th week after loading.Then MMP-1and TIMP-1 level in GCF was tested with the method of enzyme-linked immunosorbent assay.Vernier caliper was used to measuring the moving distance of canines.All data were statistically analyzed by SPSS19.0. Results 1 No matter on group A or group B,canines move fastest in 1 week after retraction,the comparisons had statistical meanings. The speed on group A and B decreased after 2 weeks and got more slow hereafter, and even declined to 0 in 5th 、6th week. The comparisons of moving distances among groups and jaws had statistical meanings(P〈0.05). They all had the largest displacement in the 1st week,,but but group B moved more evenly. 2With the concentration levels of MMP-1 and TIMP-1 in GCF before retraction set standard line, 1 week after retraction, the levels of MMP-1 and TIMP-1 in Group A and Group B on maxilla and mandible all began to increase to the maximum, and start to decline soon afterwards but Group B showed a stable even change line on both maxilla and mandible. The concentration in group A decreased rapidly after2 weeks, and slowly hereafter, and to the before-retraction levels in 6th week.
