摘 要:目的探讨钼靶X线和彩色多普勒超声单独和联合诊断对乳腺癌术前评估的价值。方法收集2011-2013年该院收治的经钼靶X线、彩色多普勒超声检查后拟诊为乳腺癌的63例患者为研究对象,均经手术病理证实。对比分析两种检查方法单独及联合应用对乳腺癌癌灶的检出率、癌灶大小的符合率、淋巴结的转移情况及临床手术方式的影响。结果钼靶X线与彩色多普勒超声联合诊断对病灶的检出率明显高于单独使用其中一种检查手段(P<0.05);彩色多普勒超声对腋窝淋巴结的检出率明显高于钼靶X线(P<0.05)。联合诊断、钼靶X线、彩色多普勒超声对病灶测值与病理测值结果进行相关分析(r值0.975、0.939、0.947)显示,联合诊断对病灶术前大小的评价更为精准。拟定27例保乳术中,实施19例,联合诊断否定6例,仅2例被术中病理否定改行根治术。结论钼靶X线和彩色多普勒超声联合应用能提高乳腺癌检出率及临床手术方案的准确性。Objective To explore the value of alone and combined application of Molybdenum target X-ray and color Doppler ul- trasound in preoperative evaluation of breast cancer. Methods Sixty-three patients presumptively diagnosed as breast cancer after Molybdenum target X-ray and color Doppler ultrasound in the hospital from 2011 to 2013 were collected as study objects, then they were diagnosed definitely by pathological examination. The effects of alone and combined application of Molybdenum target X-ray and color Doppler ultrasound on detection rate of breast cancer, the coincidence rate of tumor size, lymph node metastasis, and clinical-surgical mode were compared and analyzed. Results The detection rate of Molybdenum target X-ray combined with color Doppler ultrasound for breast cancer was statistically significantly higher than those of alone application of Molybdenum target X-ray and alone application of color Doppler ultrasound ( P〈0.05 ) ; the detection rate of color Doppler ultrasound for axillary lymph nodes was statistically significantly higher than that of Molybdenum target X-ray ( P〈0.05 ) . The correlation coefficients of detection values between combined diagnosis, Molybdenum target X-ray, color Doppler ultrasound and pathological diagnosis were 0. 975, 0. 939, and 0. 947, respectively. The evaluation of combined diagnosis was the most accurate. Among 27 patients preparing to undergoing breast conserving surgery, 19 patients underwent the surgery actually, 6 patients were negated by combined diagnosis, only two patients converted to radical mastectomy after intraoperative pathological diagnosis. Conclusion Molybdenum target X-ray combined with color Doppler ultrasound can improve the detection rate of breast cancer and the accuracy of clinical operation plan.
