异种冻干骨及与富血小板纤维蛋白的复合物修复钛螺纹钉周骨缺损的实验研究

摘　　要：目的通过将异种冻干骨及其与富血小板纤维蛋白(platelet-rich fibrin,PRF)的复合物分别植入兔下颌骨骨缺损,即刻植入钛螺纹钉的实验研究,探讨异种冻干骨及其与PRF的复合物对钛螺纹钉周围骨缺损区修复的作用。方法 12只新西兰大耳白兔,于下颌骨造极限骨缺损,并紧贴骨缺损近中及远中边缘各植入钛螺纹钉;右侧缺损区植入异种冻干骨与PRF的复合物为实验组,左侧缺损区植入异种冻干骨作对照组;2组于术后4、8、12周处死后常规取材,行大体观察、X线观察、骨密度分析、组织学观察。结果术后4、8、12周实验组钛螺纹钉骨缺损新生骨灰度值比对照组大(P<0.05)。组织学观察显示,实验组骨成熟程度均高于对照组。结论异种冻干骨可以作为一种骨替代材料修复钛螺纹钉周围骨缺损,与PRF联合应用后,在修复骨缺损中显示出了更好的成骨作用。Objective To discuss the effect of heterogeneous freeze-dried bone and its platelet-rich fibrin（PRF）compound in the repair of the bone defect around titanium screw.Methods Twelve New Zealand white rabbits received surgery as follow steps：a critical bone defect was made at the buccal side of the mandible,and the titanium screws were implanted into the bone defect near mesial and distal edge of the alveolar.In all experimental rabbits,the right side of the mandible served as the experimental group（placed heterogeneous freeze-dried bone with PRF）and the left side as the control group（placed heterogeneous freeze-dried bone）.All the rabbits of experimental group and the control group were killed separately on 4th,8th and 12 th week after implanting.Then,the gross observation,X-rays,BMD analysis and histopathology were followed.Results After operation for 4,8,12 weeks,gray values of the new bone in experimental group were higher than those of control group（P 0.05）.Histopathology observation showed the bone maturity in experimental group was higher than in control group.Conclusion Heterogeneous freeze-dried bone as a bone substitute materials can repair bone defects around titanium screw;PRF in its compound with heterogeneous freeze-dried bone plays a positive role in osteogenesis.
