摘 要:目的:研究结构脂肪乳剂(C6-24力文)对胃肠肿瘤患者静息能量代谢和脂质过氧化的影响。方法:选取2013年7月—2014年7月在我院接受治疗的胃肠道肿瘤患者82例,随机分为结构脂肪乳剂(STG)组和中长链脂肪乳剂(MCT/LCT)组,每组41例。患者在术后第2天至第6天分别采用STG和MCT/LCT构成的等氮、等热量肠外营养(PN)支持,共5 d。结果:接受手术后第6天,STG组胃肠肿瘤患者累计氮平衡值与MCT/LCT组胃肠肿瘤患者相比较低(P<0.05);STG组胃肠肿瘤患者在术后的第1天至第4天超敏C-反应蛋白(hs-CRP)水平与MCT/LCT组患者相比较低(P<0.05)。STG组胃肠肿瘤患者血清丙二醛(MDA)浓度低于MCT/LCT组患者,差异有统计学意义(P<0.05),STG组胃肠肿瘤患者谷胱甘肽过氧化物酶(GSH-Px)水平高于MCT/LCT组患者,差异有统计学意义(P<0.05)。结论:STG组胃肠肿瘤患者采用PN支持治疗所造成的机体代谢紊乱更小,胃肠肿瘤患者在减轻脂质过氧化损伤和维护能量代谢稳定方面,STG比MCT/LCT效果更好。Objective:To study the structure of fat emulsion(C6 - 24 Li Wen)effect on resting energy metabolism and lipid peroxidation in patients with gastrointestinal cancer. Methods:From 2013 July to 2014 July 82 patients with gastrointesti-nal tumor treated in hospital,which were randomly divided into structure of fat emulsion(STG)group and medium and long chain triglyceride emulsion(MCT/ LCT)group,two groups each of 41 patients. Patients in the postoperative 2 -6 days respec-tively by STG and MCT/ LCT etc. constituten,heat PN support,5 d. Results:after an operation group 6 d STG of gastrointestinal tumor patients accumulated nitrogen balance value and the MCT/ LCT group of gastrointestinal tumor patients compared to the lower(P 〈 0. 05). The level of 4 d hypersensitive C - reactive protein(hs - CRP)and MCT/ LCT groups were compared with low STG group of gastrointestinal tumor patients first after operation(P 〈 0. 05). Serum Malondialdehyde(MDA)in the pa-tients with gastrointestinal tumor concentration of STG group was lower than that of group MCT/ LCT patients of gastrointestinal tumor,statistically significant data difference(P 〈 0. 05),STG group of gastrointestinal cancer patients and the level of gluta-thione peroxidase(GSH - Px)was higher than group MCT/ LCT patients of gastrointestinal tumor,statistically significant data difference(P 〈 0. 05). Conclusion:STG group of gastrointestinal tumor patients using PN to support smaller metabolic disor-ders caused by the treatment of gastrointestinal tumor patients,in terms of reducing lipid peroxidation damage and maintain the stability of energy metabolism,STG better than MCT/ LCT.
