B细胞淋巴瘤患者外周血T淋巴细胞亚群的检测及与Ki67的关系

摘　　要：目的：了解 B 细胞淋巴瘤患者外周血 T 淋巴细胞亚群、NK 细胞的表达以及与 Ki67的关系。方法：选择 B细胞淋巴瘤患者32例，男18例，女14例，年龄36-62岁。应用流式细胞仪（FCM）检测总 T 细胞，Ts，Th，NK 细胞绝对值计数，并计算出 Th/ Ts 值，对各种数据进行比较。同时用免疫速率比浊法测定血清中免疫球蛋白。据 Ki67表达阳性率分为低表达组和高表达组，了解 Ki67与免疫功能的关系。结果：非霍奇金淋巴瘤（NHL）患者 Th 明显低于正常对照组，P 〈0.05；Ts 低于正常对照组，P 〉0.05；总 T 细胞明显低于正常对照组，P 〈0.01；Th/ Ts，NK 均明显低于正常对照组，P 〈0.01。Ki67高表达组 Th，总 T 细胞，Th/ Ts，NK 细胞明显低于正常对照组，P 〈0.05；Ts 明显高于正常对照组，P 〈0.05；Ki67低表达组 Th，总 T 细胞，Th/ Ts，NK 细胞低于正常对照组，P 〈0.01；Ts 高于正常对照组，但差异无统计学意义（P 〉0.05）。结论：B 细胞淋巴瘤患者存在细胞免疫抑制与紊乱，且基础免疫低下。Ki67表达越高，免疫功能越低；反之，免疫功能越强。Objective：To investigate the role of abnormal T lymphocyte subsets and natural killer cells,and to investigate their relationship of T lymphocyte subsets and Ki67 proliferation index round the patient with B cell lymphoma. Methods：Select the 32 cases of patient with B cell lymphoma（male 18 cases,female 14 cases）,aged 36 - 62 years old,apply the method of the fluid cell machine（FCM）to detect the T lymph cell and its subsets including total T,Th,Ts and counting of the absolute value of natural killer cells,and calculate the value of Th/ Ts,make a comparison of various data. At the same time,using the immu-nization rate nephelometry to measure the immunoglobulin in the serum. It is divided into low expression group and high ex-pression group according to the positive rate of expression of Ki67,try to understand the relationship between Ki67 and the im-mune function. Results：The Th of the patients with NHL is apparently lower than the normal contrast group（P 〈 0. 05）. The Ts is lower than the normal contrast group（ P 〉 0 . 0 5 ）. Total T is apparently lower than the normal contrast group（ P 〈 0 . 0 1 ）. Th/ Ts and NK is apparently lower than the normal contrast group（P 〈 0. 01）. Th,total T,Th/ Ts,NK cell in the high expres-sion group of Ki67 are apparently lower than the normal contrast group（P 〈 0. 05）. The Ts of the high expression of Ki67 is ap-parently higher than the normal contrast group（P 〈 0. 05）. Th,total T,Th/ Ts,NK cell in the low expression group of Ki67 are lower than the normal contrast group（P 〈 0. 05）. The Ts of the low expression of Ki67 is higher than the normal contrast group （P 〉 0. 05）. Conclusion：The patient with B cell lymphoma has the cell immunity suppression and disorder,and the basic im-munity is low. The higher the expression of Ki67,the lower the immunity;conversely,the higher the immunity.
