鼻咽癌患者治疗前外周血α-羟基丁酸脱氢酶水平与其预后的相关性分析

摘　　要：目的研究鼻咽癌患者治疗前外周血α-羟基丁酸脱氢酶（α-HBDH）水平与其预后的相关性。方法选择2008年5月至2011年10月广州医科大学附属肿瘤医院初诊的335例鼻咽癌患者作回顾性分析。收集患者在治疗前外周血旺一羟基丁酸脱氢酶数值。通过Kaplan—Meier、X2检验分析比较血α-羟基丁酸脱氢酶与鼻咽癌患者总生存率和无进展生存率的相关性。采用Cox比例风险模型来确定预后因素。结果通过X2检验发现治疗前α-HBDH水平与N分期存在相关性（P=0.03），与年龄（P=0.528）、性别（P=0.802）、T分期（P=0.619）、临床分期（P=0.519）无相关性。通过Kaplan—Meier分析和Ⅳ。检验发现，治疗前高α-HBDH组〉107.5U／L在总生存率（OS）上小于治疗前低α-HBDH组≤107.5U／L（P=0.014）。COX比例风险回归模型显示，年龄〉48岁、T3＋4期、N3期、治疗前高α-HBDH水平均与患者总生存率相关（P=0．042）。结论治疗前外周血饯一羟基丁酸脱氢酶（α-HBDH）水平与鼻咽癌总生存率相关。Objective To investigate the correlation between the level of serum α-hydroxybutyrate dehydrogenase （α-HBDH） before treatment and prognosis in patients with nasopharyngeal carcinoma. Methods 335 cases of nasopharyngeal carcinoma in our hospital from May 2008 to October 2011 were selected. Recorded the level of serum α-HBDH before treatment. Analyzed the correlation between the rates of overall survival （OS）, progression-free survival （PFS） and the level of serum α-HBDH before treatment with Kaplan-Meier method and X2 test. Identified prognostic factors with Cox proportional hazards model. Results The level of serum α-HBDH before treatment was associated with N stage （P=0.03）, but not associated with age, sex, T stage and clinical stage （P=0.528, 0.802, 0.619, 0.519）. The overall survival of patients whose serum α-HBDH before treatment was 〉107.5 U/L was lower than that of patients whose serum α-HBDH before treatment was ≤107.5 U/L （P=0.014）. Cox proportional hazards model showed thatage 〉48 years old, T3＋4 stage, N3 stage, high level of serum α-HBDH before treatment were associated with the overall survival of patients with nasopharyngeal carcinoma. Conclusion The level of serum α-HBDH before treatment is associated with the overall survival of patients with nasopharyngeal carcinoma.
