柔红霉素和去甲氧柔红霉素治疗急性白血病的安全性和有效性分析

摘　　要：目的探讨柔红霉素(DNR)和去甲氧柔红霉素(IDA)治疗急性白血的疗效和安全性。方法选择2012年1月至2014年9月间收治的急性白血病患者68例,采用随机数字表法分为对照组和观察组,每组34例。观察组患者采用IDA+阿糖胞苷或IDA+长春新碱+地塞米松化疗;对照组患者采用DNR+阿糖胞苷或DNR+长春新碱+地塞米松进行化疗。急性非淋巴细胞白血病治疗2个疗程,急性淋巴细胞白血病1个疗程后进行观察,主要观察指标为临床治疗效果及不良反应。结果观察组患者治疗后有效率为85.3%(29/34),高于对照组70.6%(24/34),差异有统计学意义(P<0.05)。观察组患者心脏毒副反应发生率为17.7%(6/34),低于对照组44.1%(6/34),差异有统计学意义(P<0.05)。结论 IDA治疗急性白血病具有较好的临床疗效,能够显著降低患者的不良反应,具有较高的安全性。Objective To explore the effect and safety of daunorubicin（ DNR） and idarubicin（ IDA） for the treatment of acute leukemia. Methods Totally 68 patients with acute leukemia were selected in The First People＇s Hospital of Zhangjiagang City from January 2012 to September 2014,they were randomly divided into the control group and the observation group by the random number table method,with 34 cases in each group. The observation group used IDA ＋ cytarabine,or DNR ＋ vincristine ＋ dexamethasone.The control group used DNR ＋ cytarabine chemotherapy,or DNR ＋ vincristine ＋ dexamethasone. Clinical treatment effect and the adverse reactions were observed after two treatment courses of acute nonlymphocytic leukemia patients and one treatment course of acute lymphoblastic leukemia patients. Results Response rate of observation group was 85. 3%,higher than the control group（ 70. 6%）,with significant difference（ P 0. 05）. The cardiac adverse reaction occrance rate of the observation group was 17. 7%,lower than the control group（ 44. 1%）,and the difference was statistically significant（ P 0. 05）. Conclusion It has better clinical curative effect for IDA treatment of acute leukemia,which can significantly reduce the adverse reaction of patients,and has higher security.
