脉冲Er:YAG激光和涡轮机对牙本质粘接界面的超微结构观察和抗剪切强度分析

摘  要:目的:采用脉冲铒钇铝石榴石(Er:YAG)激光和涡轮机分别处理牙本质,使用不同粘接系统,比较两种方式对牙本质的粘接效果。方法:选取新鲜拔除的人下颌第三磨牙,按不同的处理方法和粘接系统随机分为5组。a:激光+自酸蚀;b:激光+磷酸+全酸蚀;c:激光+全酸蚀;d:涡轮机+自酸蚀;e:涡轮机+磷酸+全酸蚀。应用万能力学测试仪测其抗剪切强度,扫描电子显微镜观察其超微形态结构。结果:不同预备方法相比,a组与d组间抗剪切强度有明显差异(p<0.05,a>d),b组与e组间抗剪切强度无差异(p>0.05)。对于Er:YAG激光处理组,自酸蚀系统及酸蚀冲洗系统所得抗剪切强度值较高,分别为(16.94±2.96)MPa、(14.39±2.74)MPa;单独全酸系统所得抗剪切强度值最低为(9.44±1.89)MPa。结论:Er:YAG激光处理牙本质后其粘接效果不低于涡轮机组;Er:YAG激光处理牙本质后采用自酸蚀系统及磷酸+全酸蚀系统粘接效果均较好。Objective:This study aimed to compare bonding effects of different adhesive systems bonded to Er: YAG laser-or bur prepared dentin surfaces. Method: Freshly extracted human third molars were selected and randomly divided into five groups according to different preparation methods and bonding systems, a: Er: YAG laser + self-etching adhesive; b:Er:YAG laser + acid etching+ total-etching adhesive; c : Er : YAG laser + only total-etching adhesive; d : bur+ self-etching adhesive; e : bur+ acid etching+ total-etching adhesive. Then a universal testing machine was used to measure shear bond strengths(SBS), and a scanning electron microscopy was applied to observe the uhrastructures of bonding interfaces. Results: Comparing the shear bond strengths of groups with different preparation methods: There were significant differences in SBS values between group a and group d (p〈0.05 ,a〉d), no significant differences between group b and group e (p〉0.05). Among Er:YAG la- ser-prepared dentin groups, self-etching adhesive system (16. 94 ± 2. 96)and acid etching + total-etching adhesive system (14.39±2.74)achieved higher SBS values(in MPa) ,and total-etching adhesive system(9.44±1.89) had the lowest SBS value. Conclusions:Bonding effect of Er=YAG laser-prepared dentin was not lower than bur-prepared. The self-etch adhesive system performed as well as the acid etch+ total-etch adhesive system on bonding effects after Er: YAG laser preparation.
