GnRH-a联合LNG-IUS对子宫内膜异位症Ⅲ-Ⅳ期患者术后疼痛的疗效

摘　　要：目的:探讨促性腺激素释放激素激动剂(GnRH-a)联合左炔诺孕酮宫内缓释系统(LNG-IUS)对子宫内膜异位症Ⅲ-Ⅳ期患者术后疼痛的治疗效果。方法:回顾性分析100例已行保守性腹腔镜手术的子宫内膜异位症Ⅲ-Ⅳ期患者,给予GnRH-a联合LNG-IUS治疗的患者50例(实验组);给予单纯GnRH-a治疗的患者50例(对照组)。术后6个月、12个月随访患者疼痛评分(VAS法,B&B法)。结果:两组内术后6个月、12个月VAS、B&B评分显著低于术前(P<0.05)。对照组术后12个月VAS、B&B评分显著高于术后6个月(P<0.05)。实验组术后12个月VAS、B&B评分显著低于对照组(P<0.05)。结论:GnRH-a联合LNGIUS对腹腔镜保守性手术术后子宫内膜异位症Ⅲ-Ⅳ期患者的疼痛治疗作用确切、长效,值得临床推广。Objective：To observe the efficacy of GnRH analogue combined with LNG - IUS on pelvic pain of stageⅢ-Ⅳendometriosis patients after conservative laparoscopic surgery. Methods：A total of 100Ⅲ-Ⅳ stage endome- triosis cases, after laparoscopic diagnosis and conservative laparoscopic surgery ,50 in experimental group and treated with GnRH analogue combined with LNG -IUS,50 in control group and treated with GnRH analogue. Retrospectively analyzed the pain score of 6 and 12 months postoperation by VAS test and B＆B test. Results：In two groups, the VAS score and B＆Bscore of 6,12 months postoperation were significantly lower than thatof preoperation （P 〈 0.05 ）. The VAS score and B＆B score of 12 months postoperation were significantly higher than that of 6 months in control group （ P 〈 0.05 ）. The VAS score and B＆B score of experimental group was significantly lower than that of control groupin 12 months postoperation（P 〈 0.05 ）. Conclusion： GnRH analogue combined with LNG- IUS are effective and have long effect, and they are a promising treatment of endometrosis.
