加速康复外科在45例直肠癌保肛围手术期的应用分析

摘　　要：目的探讨加速康复外科治疗直肠癌保肛术的疗效及安全I生，为临床提供参考。方法选取本院2013年1月～2015年6月间收治的直肠癌保肛术患者90例作为研究对象，根据围手术期处理措施的不同，分为加速康复组和传统治疗组，对两组患者的疗效及并发症的发生率进行比较。结果对加速康复组和传统治疗组的肠蠕动恢复时间、术后镇痛时间、进食时间、术后住院天数及平均住院费用等指标进行比较，差异有统计学意义（P〈0．05）；加速康复组术后并发症的发生率为11．11％（5／45），传统康复组术后并发症的发生率为31．11％（14／45），两组比较差异有统计学意义（P〈0．05）；治疗前，两组患者人血白蛋白、血红蛋白、C-反应蛋白（CRP）等指标比较，差异无统计学意义（P〉0．05）；治疗后，两组患者比较，差异有统计学意义（P〈0．05）；加速康复组治疗前后，差异无统计学意义（P〉0.05）。结论对直肠癌保肛术患者实施加速康复治疗安全、有效，有助于患者早期恢复。Objective To discuss the efficacy and safety of fast track surgery in rectal cancer reserving anus operations and to provide theoretical basis for clinical practice. Methods Ninety patients experienced rectal cancer reserving anus operation in our hospital from Jan 2013 to Jun 2015 were selected and divided into fast track group and traditional group according to different perioperative management. The efficacy and incidence of complications of the two groups were compared. Results Compared to the traditional group, the fast track group showed statistically significant differences in recovery time of intestine peristalsis, analgesia duration after operation, time of taking food, hospital stay after operation and average hospitalization expenses （P〈0.05）. The incidence of complications in the fast track group was 11.11% （5/45）, while that in the traditional group was 31.11%（14/45）, which was statistically significant（P〈0.05）. Before treatment, there was no statistically significant difference between the two groups in human blood albumin, hemoglobin, and CRP （P〉0.05）, while after treatment, these indicators showed statistically significant difference（P〈0.05）. Within the fast track group, there was no statistically significant difference before and after treatment（P〉0.05）. Conclusion Fast track surgery in rectal cancer reserving anus operations is safe and effective and is conducive to the early recovery of patients.
